益肾通络蠲疲汤治疗肾虚血瘀型癌因性疲乏临床疗效观察OA
Clinical observation on the efficacy of Yishen Tongluo Juanpi Decoction in treating cancer-related fatigue of kidney deficiency and blood stasis type
目的:观察益肾通络蠲疲汤治疗肾虚血瘀型癌因性疲乏(CRF)的临床疗效.方法:选取2022年3月—2023年8月期间在山西省中西医结合医院老年病科一病区和肿瘤科诊断为CRF的62例患者,采用随机数字表法分为对照组与研究组,每组31例.对照组施以基本支持治疗及对症治疗,研究组在对照组常规治疗基础上加服益肾通络蠲疲汤.两组均持续治疗3个月.采用综合评价方法,包括人体功能状况卡式评分(KPS)、Piper疲乏修订量表(PFS)以及中医证候积分,检测患者血常规和肝肾功能,记录不良反应,评价治疗的安全性,全面衡量治疗效果.结果:治疗后,两组患者KPS评分均提高,且研究组评分高于对照组,差异有统计学意义(P<0.05,P<0.01).两组患者PFS评分各项指标(情感、躯体、认知、行为及总分)和中医证候积分均降低,且研究组各项评分低于对照组,差异均有统计学意义(P<0.05,P<0.01).研究组患者PFS评分疗效总有效率(83.87%)明显高于对照组(41.93%),且临床治愈率(80.65%)明显高于对照组(45.16%),差异均具有统计学意义(P<0.01).两组患者血常规、肝功能指标均正常,且均无不良反应发生.结论:益肾通络蠲疲汤在CRF治疗中的卓越疗效及安全特性,能明显减轻癌症相关疲劳,提升生活质量,为中医学在CRF治疗领域的临床应用提供了坚实证据,预示其在这一领域具有广泛的应用潜力和市场价值.
Objective:To observe the clinical efficacy of Yishen Tongluo Juanpi Decoction in treating cancer-related fatigue(CRF)of kidney deficiency and blood stasis type.Methods:A total of 62 patients diagnosed with CRF in the first in-patient ward of Geriatrics Department and Oncology Department of Shanxi Provincial Integrated TCM and WM Hospital from March 2022 to August 2023 were selected and randomly divided into a control group and a study group using a random num-ber table,with 31 patients in each group.Patients in the control group received basic supportive treatment and symptomatic treatment,while patients in the study group were additionally administered Yishen Tongluo Juanpi Decoction on the basis of the control group's conventional treatment.Both groups were treated continuously for 3 months.A comprehensive evaluation method,including the Karnofsky Performance Status(KPS)score,Piper Fatigue Scale-Revised(PFS),and traditional Chinese medicine(TCM)syndrome scores,was adopted to test the patients'blood routine and liver and kidney functions,record ad-verse reactions,evaluate the safety of treatment,and comprehensively measure the therapeutic effect.Results:After treat-ment,the KPS scores of both groups improved,and the scores of the study group were higher than those of the control group,with statistically significant differences(P<0.05,P<0.01).The PFS scores(emotional,physical,cognitive,behavioral and total scores)and TCM syndrome scores of both groups decreased,and the scores of the study group were lower than those of the con-trol group,with statistically significant differences(P<0.05,P<0.01).The total effective rate of PFS score efficacy in the study group(83.87%)was significantly higher than that in the control group(41.93%),and the clinical cure rate(80.65%)was sig-nificantly higher than that in the control group(45.16%),with statistically significant differences(P<0.01).The blood routine and liver function indicators of patients in both groups were normal,and no adverse reactions occurred.Conclusion:Yishen Tongluo Juanpi Decoction demonstrates excellent efficacy and safety characteristics in the treatment of CRF,significantly al-leviating cancer-related fatigue and improving quality of life.This provides solid evidence for the clinical application of TCM in the field of CRF treatment,indicating its broad application potential and market value in this area.
