扶正安中汤联合奥沙利铂、氟尿嘧啶与亚叶酸钙方案治疗晚期胃癌的疗效观察OA
Observation on the efficacy of Fuzheng Anzhong Decoction combined with oxaliplatin,fluorouracil and leucovorin regimen in the treatment of advanced gastric cancer
目的:观察扶正安中汤联合奥沙利铂、氟尿嘧啶与亚叶酸钙(FOLFOX4)方案治疗晚期胃癌的疗效和安全性.方法:选取安徽中医药大学第一附属医院肿瘤二科2024年2月—2025年2月确诊为晚期胃癌的患者,按随机数字表法分为对照组和治疗组,每组40例.对照组以FOLFOX4方案治疗(简称:化疗),治疗组以扶正安中汤联合FOLFOX4方案治疗.21 d为1个疗程,两组患者均治疗3个疗程.观察两组患者临床疗效、治疗前后卡氏评分(KPS)改善情况、癌胚抗原(CEA)、糖类抗原19-9(CA199)、糖类抗原72-4(CA724)等肿瘤标志物水平,并记录化疗不良反应发生情况.结果:治疗组患者临床有效率(87.5%)明显高于对照组(67.5%),差异有统计学意义(P<0.05).治疗后,治疗组KPS评分升高,且高于对照组,差异均有统计学意义(P<0.05,P<0.01).治疗后,两组患者CEA、CA199、CA724等肿瘤标志物水平均低于治疗前,且治疗组患者各项肿瘤标志物水平改善情况均优于对照组,差异均有统计学意义(P<0.05,P<0.01).治疗组患者恶心呕吐、食欲下降、神经毒性、白细胞降低、腹泻等不良反应发生率均低于对照组,差异均有统计学意义(P<0.05).结论:扶正安中汤联合FOLFOX4化疗方案可显著增强晚期胃癌患者的治疗效果,提高KPS功能状态评分,降低CEA、CA199、CA724浓度,并有效减少化疗相关不良反应的发生.
Objective:To observe the efficacy and safety of Fuzheng Anzhong Decoction combined with the oxaliplatin,fluorouracil and leucovorin(FOLFOX4)regimen in the treatment of advanced gastric cancer.Methods:Patients diagnosed with advanced gastric cancer in the Second Department of Oncology,the First Affiliated Hospital of Anhui University of Chinese Medicine from February 2024 to February 2025 were selected and randomly divided into a control group and a treatment group according to the random number table method,with 40 cases in each group.Patients in the control group received chemotherapy with the FOLFOX4 regimen,while patients in the treatment group received Fuzheng Anzhong Decoc-tion combined with the FOLFOX4 regimen.A treatment cycle was 21 days,and both groups completed three cycles.The clinical efficacy,Karnofsky Performance Status(KPS)score improvement,levels of tumor markers including carcinoembry-onic antigen(CEA),carbohydrate antigen 19-9(CA199)and carbohydrate antigen 72-4(CA724)before and after treat-ment,as well as the incidence of chemotherapy-related adverse reactions,were observed in both groups.Results:The clini-cal effective rate in the treatment group(87.5%)was significantly higher than that in the control group(67.5%),and the dif-ference was statistically significant(P<0.05).After treatment,the KPS score in the treatment group increased and was higher than that in the control group,with statistically significant difference(P<0.05,P<0.01).After treatment,the levels of tu-mor markers,including CEA,CA199 and CA724,in both groups were lower than those before treatment,and the improve-ment in tumor marker levels in the treatment group was superior to that in the control group,with statistically significant dif-ferences(P<0.05,P<0.01).The incidence rates of adverse reactions such as nausea and vomiting,decreased appetite,neuro-toxicity,leukopenia and diarrhea in the treatment group were lower than those in the control group,with statistically signifi-cant differences(P<0.05).Conclusion:Fuzheng Anzhong Decoction combined with the FOLFOX4 chemotherapy regimen can significantly enhance the therapeutic effect in patients with advanced gastric cancer,improve the KPS functional status score,reduce the concentrations of CEA,CA199 and CA724,and effectively reduce the occurrence of chemotherapy-related adverse reactions.
