ICU护士疼痛管理循证护理实践行为意向的质性研究OA
Qualitative Study on Behavioral Intention of Evidence-based Nursing Practice in Pain Management Among ICU Nurses
目的 了解ICU护士对疼痛管理循证实践的想法,为促进高质量的ICU疼痛管理实践提供参考依据.方法 运用现象学研究法,采用目的抽样法,选取贵阳市某三级甲等医院ICU护士14名作为研究对象,进行半结构式访谈,采用Colaizzi七步分析法对资料进行编码、分类和提炼主题.结果 基于多理论模型中涉及的启动和维持两个阶段,共提炼出以下6个主题及16个亚主题:参与性谈话(效能导向的价值认同、规范与现实的实践冲突导致的挫折感、投入产出失衡的无力感),行为自信(知识储备不足的效能缺失感、共情支撑的效能补偿感),物质环境改变(工具不适与管理缺位的困扰感、对环境优化的期盼),情感转变(挫败中的反思成长、决策缺失下的专业疏离感、价值实现的内生满足感),行为实践改变(理论与实践脱节的无力感、对外部监督反馈的依赖感),社会环境改变(对专业组织支持的期待、对制度与执行脱节的困惑、领导与团队协作的决定性作用、可持续学习的资源困境).结论 ICU护士普遍持有积极的疼痛管理态度,并渴望在实践中发挥更大的主动作用,但在过程存在较多阻碍影响因素.这提示护理管理者需要重视并优化疼痛管理制度,加强ICU护士在疼痛管理方面的培训,以促进疼痛管理循证护理实践.
Objective To explore ICU nurses'perspectives on evidence-based practice in pain management,aiming to provide a reference for promoting high-quality pain management practice in the ICU setting.Methods Using a phenomenological approach,14 ICU nurses were selected from a Grade Ⅲ-A hospital in Guiyang by purposive sampling for semi-structured interviews.Their interview data were coded,categorized and extracted for themes using Colaizzi's seven-step method.Results Based on the initiation and maintenance stages in the multi-theoretical model,a total of 6 themes and 16 sub-themes were extracted:1)participatory dialogue(efficacy-oriented value identity;sense of frustration due to conflict between norms and real-world practice;sense of powerlessness due to input-output imbalance),2)behavioral confidence(perceived lack of efficacy due to insufficient knowledge;perceived efficacy compensation supported by empathy),3)changes in physical environment(distress caused by inadequate tools and lack of management;anticipation of environmental optimization),4)emotional shift(reflective growth amidst setbacks;professional detachment due to lack of decision-making;intrinsic satisfaction derived from value realization),5)changes in behavioral practice(sense of powerlessness due to theory-practice gap;sense of dependence on external supervision and feedback),and 6)changes in social environment(expectations for support from professional organizations;confusion about policy-execution gap;decisive role of leadership and teamwork;resource dilemmas for sustainable learning).Conclusion ICU nurses generally hold positive attitudes towards pain management and express a strong desire to play a more proactive role in practice.However,numerous barriers and influencing factors hinder this process.These findings suggest that nursing administrators should emphasize and optimize pain management systems and strengthen pain management training for ICU nurses to facilitate evidence-based nursing practice in this area.
