基于铁死亡途径和"肝肾阴虚"探析缺血性脑卒中病理机制及理论内涵OA

This review uses the mechanisms of ferroptosis and the theory of liver-kidney Yin deficiency as entry points to provide an interdisciplinary explanation of the pathological basis of ischemic stroke(IS).Ferroptosis,as a novel form of cell death driven by iron-dependent lipid peroxidation,plays a crucial role in neuronal damage during IS.Its core mechanisms involve intracellular iron overload,imbalance of the antioxidant defense system,and accumulation of lipid peroxidation products.Traditional Chinese medicine(TCM)classifies IS as"stroke",and its fundamental pathogenesis lies in liver-kidney Yin deficiency,where Yin fails to control Yang,leading to malnourishment of the marrow and internal wind.This study proposes,through mechanism comparison and theoretical analysis,that the"Yin deficiency-internal heat excess"state presented by the core pathogenesis of liver-kidney Yin deficiency may highly coincide with the decreased antioxidant capacity,iron homeostasis disorder,and enhanced oxidative stress revealed by modern medicine,thus inducing ferroptosis.This interdisciplinary connection provides a new direction for explaining the pathogenesis-symptom correspondence of IS and provides theoretical support for the integrated traditional Chinese and Western medicine intervention of multi-target regulation of ferroptosis by TCM.
