窄谱成像放大内镜联合色素内镜与单一窄谱成像放大内镜在结直肠早期肿瘤检查中的应用对比研究OA
Comparison of Magnifying Endoscopy with Narrow-Band Imaging(ME-NBI)Combined with Chromoendoscopy versus ME-NBI Alone for the Ex-amination of Early Colorectal Tumors
目的:自身对照分析窄谱成像放大内镜联合色素内镜与单一窄谱成像放大内镜在结直肠早期肿瘤检查中的应用效果.方法:选取2023 年7 月至2024 年7 月在成都市青白江区人民医院结肠镜检查中发现的232 个结直肠早期肿瘤病变为研究对象,所有观察到的病变均使用窄谱成像放大内镜(单一组)和窄谱成像放大内镜联合色素内镜(联合组)进行检查.以病理结果作为标准,分别对比两组的诊断效能.结果:窄谱成像放大内镜联合色素内镜与单一窄谱成像放大内镜在不同大小病变中的诊断准确度无差异(P>0.05),联合组和单一组pit pattern 分型诊断结果与病理诊断结果一致性好(Kappa 值为 0.783 和 0.758,P<0.05),在高风险早期肿瘤性病变诊断中一致性满意(Kappa 值为 0.893,P<0.05),两组诊断的敏感度均为 92.5%,特异度分别为 78.4%和 75.3%,阳性预测值分别为96.9%和95.6%,阴性预测值分别为85.3%和82.8%,准确度分别为92.2%和91.4%(P 均>0.05).结论:单一窄谱成像放大内镜与窄谱成像放大内镜联合色素内镜两种方式具有基本一致的诊断准确度,在结直肠早期肿瘤检查中窄谱成像放大内镜可以作为基石,疑难病例可使用窄谱成像放大内镜联合色素内镜.
Objective:To conduct a self-controlled study on the diagnostic efficacy of magnifying endoscopy with narrow-band imaging(ME-NBI)combined with chromoendos-copy(CE)compared with ME-NBI alone in the detection and diagnosis of early colorectal tumors.Methods:A total of 232 early colorectal lesions identified via colonoscopy at Qingbaijiang District People's Hospital of Chengdu between July 2023 and July 2024 were included in this study.All lesions were evaluated by ME-NBI alone(ME-NBI group)and ME-NBI com-bined with CE(combined group).The pathological findings were used as the gold standard to evaluate the diagnostic per-formance of the two groups.Results:There was no significant difference between the two groups in diagnostic accuracy for lesions of different sizes(P>0.05).Pit pattern classification showed good agreement with pathological diagnosis in both the combined group and the ME-NBI group(Kappa values:0.783 and 0.758,P<0.05).Satisfactory agreement was also ob-served for the diagnosis of high-risk early neoplastic lesions(Kappa=0.893,P<0.05).The diagnostic sensitivity was 92.5%for both groups.The specificity was 78.4%for the combined group and 75.3%for the ME-NBI group.The positive predictive values were 96.9%and 95.6%,and the negative predictive values were 85.3%and 82.8%,respectively.The accuracy was 92.2%for the combined group and 91.4%for the ME-NBI group(all P>0.05).Conclusion:The two groups demonstrated largely consistent diagnostic accuracy.In the examination of early colorectal tumors,ME-NBI can serve as the cornerstone,while challenging cases may require its combination with CE.
