红藤-败酱草药对及其活性成分治疗盆腔炎性疾病后遗症作用机制研究OA
Research on the Mechanism of Action of Hongteng(Sargentodoxae Caulis)-Baijiangcao(Herba Patriniae)and Its Active Ingredients in the Treatment of Sequelae of Pelvic Inflammatory Disease
盆腔炎性疾病后遗症(sequelae of pelvic inflammatory disease,SPID)是女性盆腔炎(pelvic inflammatory disease,PID)迁延不愈、反复发作导致的慢性病理状态,中医认为湿热瘀结是SPID的关键病机.红藤-败酱草药对是以"清热解毒、祛湿化瘀"为核心治法的代表药对,在SPID治疗中发挥多途径、多靶点的调控作用.该文梳理近年来红藤-败酱草药对临床及实验类相关文献,系统综述其活性成分及相关机制,指出该药对可通过抗炎与免疫调节、抗氧化应激、抗纤维化与组织修复以及改善血液流变学等多个环节协同起效.具体而言,槲皮素、木犀草素等成分能够抑制核因子κB(nuclear factor kappa-B,NF-κB)、磷脂酰肌醇3-激酶(phosphoinositide 3-kinase,PI3K)/蛋白激酶B(protein kinase B,Akt)等信号通路,降低肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)等炎症因子水平,调节巨噬细胞极化,恢复免疫稳态;大黄素和红景天苷则通过激活核因子E2相关因子2(nuclear factor erythroid 2-related factor 2,Nrf 2)/血红素加氧酶-1(heme oxygenase-1,HO-1)信号通路,增强超氧化物歧化酶(superoxide dismutase,SOD)活性,减少丙二醛(malondialdehyde,MDA)含量,缓解氧化损伤;在抑制纤维化方面,木犀草素、原花青素等可干预转化生长因子-β1(transforming growth factor-β1,TGF-β1)/Smad和Hippo/YAP信号通路,减少细胞外基质沉积,延缓病理进程;此外,槲皮素和绿原酸还具有抗血小板聚集、调节血管活性等功能,从而改善盆腔微循环.通过对近些年红藤、败酱草活性成分及作用机制研究现状进行总结,旨在为红藤-败酱草药对临床应用的深入探索提供了理论支撑.
Sequelae of pelvic inflammatory disease(SPID)is a chronic pathological condition resulting from prolonged or recurrent pelvic inflammatory disease(PID).In traditional Chinese medicine(TCM),dampness-heat and blood stasis are considered the key pathogenesis of SPID.The herb pair Hongteng(Sargentodoxae Caulis)-Baijiangcao(Herba Patriniae),which embodies the therapeutic principle of"clearing heat-toxin,removing dampness,and resolving stasis"plays a multi-target and multi-pathway regulatory role in treating SPID.This article reviews recent clinical and experimental studies on this herb pair and systematically summarizes its active components and mechanisms.It demonstrates that the herb pair exerts synergistic effects through anti-inflammatory immunomodulation,antioxidative stress,anti-fibrosis,tissue repair,and improvement of hemorheology.Specifically,components such as quercetin and luteolin inhibit signaling pathways like NF-κB and PI3K/Akt,reduce levels of inflammatory factors including TNF-α and IL-6,modulate macrophage polarization,and restore immune homeostasis.Emodin and salidroside alleviate oxidative damage by activating the Nrf2/HO-1 pathway,enhancing SOD activity,and reducing MDA content.In terms of inhibiting fibrosis,luteolin and procyanidins interfere with the TGF-β1/Smad and Hippo/YAP signaling pathways,reduce extracellular matrix deposition,and delay pathological progression.Additionally,quercetin and chlorogenic acid inhibit platelet aggregation and regulate vascular activity,thereby improving pelvic microcirculation.This review summarizes recent advances in the study of active components and mechanisms of Hongteng(Sargentodoxae Caulis)and Baijiangcao(Herba Patriniae),providing a theoretical foundation for further exploration of their clinical application.
