目的 探讨"标本配穴"针刺法联合盐酸舍曲林对慢性不可预见性应激(chronic unpredictable mild stress,CUMS)诱导的小鼠抑郁样行为及下丘脑神经功能的改善作用,评估针药协同干预的优势,为中医"标本兼治"理论的现代神经生物学提供一定依据.方法 将30只健康ICR小鼠(雌雄各半)随机分为空白组、模型组、盐酸舍曲林干预组、标本配穴针刺法干预组、标本配穴针刺法联合盐酸舍曲林干预组,每组6只.除空白组外,其余各组均采用CUMS方法造模,干预周期为21 d.行为学方面,采用强迫游泳实验及旷场实验评估小鼠抑郁与焦虑样行为;分子指标方面,通过高效液相色谱-串联质谱法(HPLC-MS/MS)检测下丘脑γ-氨基丁酸(GABA)及谷氨酸(Glu)含量及比值,ELISA法检测血清5-羟色胺(5-HT)、去甲肾上腺素(NE)、多巴胺(DA)水平,Western blot法检测GRN2B、GRM5、VGLUT1、GABAAR和GABABR蛋白表达;组织病理与神经活动通过HE染色及FosB免疫组化分析.结果 (1)行为学改善.CUMS造模显著诱导了小鼠的抑郁样行为,表现为强迫游泳实验中的游泳不动时间延长,以及旷场实验中自主探索能力的下降.各干预组上述抑郁表型均有所改善,其中标本配穴针刺法联合盐酸舍曲林干预组的效果最为显著,其行为学表现最接近空白组.(2)神经递质水平调节.与空白组比较,模型组小鼠下丘脑GABA含量、GABA/Glu比值及血清5-HT、DA、NA水平均显著降低,而下丘脑Glu含量升高.标本配穴针刺法联合盐酸舍曲林干预能最有效地恢复神经递质的平衡,其调节作用优于单一的针刺或西药干预.(3)相关蛋白表达恢复.与空白组比较,模型组下丘脑中GRN2B、GRM5、VGLUT1、GABAAR及GABABR蛋白表达量均显著下调.标本配穴针刺法联合盐酸舍曲林干预组能显著上调这些蛋白的表达水平,恢复程度在各干预组中最为突出,呈现出明显的协同优势.(4)组织病理与神经活动提升.与空白组比较,模型组下丘脑神经元排列稀疏且出现胞体萎缩等损伤,同时FosB表达显著受抑制.标本配穴针刺法联合盐酸舍曲林干预组在改善下丘脑组织病理性损害及诱导FosB水平回升方面表现最佳,有助于重建下丘脑功能的稳态.结论 "标本配穴"针刺法联合盐酸舍曲林可显著改善CUMS小鼠抑郁样行为,恢复神经递质及相关蛋白表达平衡,上调FosB水平,改善下丘脑组织损伤,体现针药协同的抗抑郁优势,体现中医"标本兼治"在抗抑郁治疗中的转化潜力.

Obj ective To investigate the improving effect of"syndrome-essence acupoint compatibility"acupuncture combined with sertraline hydrochloride on depression-like behaviors and hypothalamic neural function in mice induced by chronic unpredictable mild stress(CUMS),evaluate the advantages of synergistic acupuncture-medicine intervention,and provide a certain modern neurobiological basis for the traditional Chinese medicine(TCM)theory of treating both manifestation and root cause.Methods Thirty healthy ICR mice(half male and half female)were randomly divided into 5 groups:blank group,model group,sertraline hydrochloride intervention group,"syndrome-essence acupoint compatibility"acupuncture intervention group,and"syndrome-essence acupoint compatibility"acupuncture combined with sertraline hydrochloride intervention group,with 6 mice in each group.Except for the blank group,mice in the other groups were subjected to CUMS modeling,and the intervention lasted for 21 days.For behavioral assessment,forced swimming test and open field test were used to evaluate depression-like and anxiety-like behaviors in mice.For molecular indicators,the contents and ratio of γ-aminobutyric acid(GABA)and glutamate(Glu)in the hypothalamus were detected by HPLC-MS/MS;serum levels of 5-hydroxytryptamine(5-HT),dopamine(DA)and norepinephrine(NA)were measured by ELISA method;the protein expressions of GRN2B,GRM5,VGLUT1,GABAAR and GABABR were detected by Western blot method.Histopathological changes and neural activity were analyzed by H&E staining and FosB immunohistochemistry.Results(1)Behavioral improvement.CUMS modeling significantly induced depression like behavior in mice,manifested as prolonged swimming immobility time in forced swimming experiments and decreased autonomous exploration ability in open field experiments.All intervention groups showed improvement in the depression phenotype mentioned above,with the most significant effect observed in the group treated with acupoint acupuncture combined with sertraline hydrochloride intervention,and their behavioral manifestations were closest to those of the blank group.(2)Regulation of neurotransmitter levels.Compared with the blank group,the model group mice showed a significant decrease in hypothalamic GABA content,GABA/Glu ratio,and serum 5-HT,DA,and NA levels,while the hypothalamic Glu content increased.The"syndrome-essence acupoint compatibility"acupuncture combined with sertraline hydrochloride intervention can most effectively restore the balance of neurotransmitters,and its regulatory effect is superior to single acupuncture or Western medicine intervention.(3)Restoration of related protein expression.Compared with the blank group,the expression levels of GRN2B,GRM5,VGLUT1,GABAAR,and GABABR proteins in the hypothalamus of the model group were significantly downregulated.The"syndrome-essence acupoint compatibility"acupuncture combined with sertraline hydrochloride intervention group can significantly upregulate the expression levels of these proteins,and the degree of recovery is most prominent among all intervention groups,showing a significant synergistic advantage.(4)Organizational pathology and increased neural activity.Compared with the blank group,the model group showed sparse arrangement of hypothalamic neurons and damage such as cell body atrophy,while FosB expression was significantly inhibited.The"syndrome-essence acupoint compatibility"acupuncture combined with sertraline hydrochloride intervention group showed the best performance in improving pathological damage to hypothalamic tissue and inducing a rise in FosB levels,which helps to rebuild the homeostasis of hypothalamic function.Conclusion The"syndrome-essence acupoint compatibility"acupuncture combined with sertraline hydrochloride intervention can significantly improve depression like behavior in CUMS mice,restore the balance of neurotransmitter and related protein expression,upregulate FosB levels,improve hypothalamic tissue damage,demonstrate the antidepressant advantages of acupuncture and medicine synergy,and demonstrate the potential for transformation of traditional Chinese medicine's"specimen and treatment"approach in antidepressant treatment.
