基于pc-tNGS技术检测支气管肺泡灌洗液中病原微生物协助病理诊断的价值OA
Value of probe capture-based targeted next-generation sequencing for detecting pathogenic microorganisms in bronchoalveolar lavage fluid
目的 评估探针捕获靶向下一代测序(probe capture-based targeted next-generation sequencing,pc-tNGS)技术检测支气管肺泡灌洗液(bronchoalveolar lavage fluid,BALF)中病原微生物协助病理诊断的价值.方法 收集39例肺部感染患者的BALF样本,采用pc-tNGS技术检测微生物的滴度和类型,并与常规微生物学检测及超多重PCR靶向下一代测序(multiplex PCR-based targeted next-generation sequencing,mp-tNGS)检测结果进行比较.结果 pc-tNGS的病原体检出率显著高于常规微生物学检测(97.44%vs 74.36%,P<0.01),并显著提高了真菌、细菌的检出率(76.92%vs 7.69%,P<0.01).pc-tNGS与mp-tNGS的病原体检出率相近(97.44%vs 97.30%,P>0.05),但两者对不同种类病原体的检测灵敏度存在差异:mp-tNGS对肺炎支原体、念珠菌的检测灵敏度更高,而pc-tNGS在检测分枝杆菌方面更具优势.此外,pc-tNGS还能同步检出BALF样本中的病原体耐药基因与毒力基因,为临床用药及后续治疗提供参考.结论 pc-tNGS是一个有效地在BALF样本中检测病原体的方法,其检测效能优于传统微生物学方法,并能提供比mp-tNGS更全面的分枝杆菌覆盖.该技术可为临床提供更精准、全面和经济的病原学诊断信息,提高肺感染性疾病的诊断水平.
Objective To evaluate the diagnostic value of probe capture-based targeted next-generation sequenc-ing(pc-tNGS)for detecting pathogenic microorganisms in bronchoalveolar lavage fluid(BALF)from patients with pul-monary infections.Methods BALF samples were collected from 39 patients with pulmonary infections.The micro-bial load and pathogen spectrum were analyzed using pc-tNGS,and the results were compared with those obtained by conventional microbiological testing(CMT)and multiplex PCR-based targeted next-generation sequencing(mp-tNGS).Results The pathogen detection rate of pc-tNGS was significantly higher than that of CMT(97.44%vs 74.36%,P<0.01).In particular,pc-tNGS markedly improved the detection rates of fungi and bacteria compared with CMT(76.92%vs 7.69%,P<0.01).The overall pathogen detection rate of pc-tNGS was comparable to that of mp-tNGS(97.44%vs 97.30%,P>0.05).However,differences in sensitivity for specific pathogens were observed:mp-tNGS showed higher sensitivity for Mycoplasma pneumoniae and Candida species,whereas pc-tNGS demonstrated superior performance in detecting Mycobacterium species.In addition,pc-tNGS simultaneously identified antimicro-bial resistance genes and virulence genes in BALF samples,providing useful information for clinical antimicrobial therapy and subsequent management.Conclusion pc-tNGS is an effective method for pathogen detection in BALF samples from patients with pulmonary infections.Compared with conventional microbiological methods,it provides higher detection efficiency and broader coverage of Mycobacterium species than mp-tNGS.This technique can provide more accurate,comprehensive,and cost-effective etiological diagnostic information,thereby improving the diagnostic efficiency of pulmonary infectious diseases.
