经支气管肺冷冻活检在CTD-ILD诊断中的应用价值:基于2025年ERS/ATS IP新分类更新的回顾性研究OA
Diagnostic value of transbronchial lung cryobiopsy in connective tissue disease-associated interstitial lung disease:a retrospective study based on the 2025 update of the ERS/ATS classification of interstitial pneumonias
目的 探讨经支气管肺冷冻活检(transbronchial lung cryobiopsy,TBLC)在结缔组织病相关间质性肺疾病(connective tissue disease-associated interstitial lung disease,CTD-ILD)诊断中的价值,并基于2025年欧洲呼吸学会(European Respiratory Society,ERS)与美国胸科学会(American Thoracic Society,ATS)间质性肺炎(interstitial pneumonia,IP)分类更新,分析其病理特征.方法 回顾性分析195例接受TBLC的弥漫性肺病变患者,其中29例经临床-影像-病理(clinico-radiologic-pathologic,CRP)综合评估体系确诊为CTD-ILD.收集患者人口学资料、临床特征及TBLC病理学结果.结果 29例CTD-ILD患者占同期行TBLC检查的弥漫性肺病变患者的14.87%(29/195),主要临床表现为咳嗽(62.07%)和呼吸困难(51.72%).病理类型呈现异质性且常为混合模式.机化性肺炎(organizing pneumonia,OP)(28.21%)和非特异性间质性肺炎(nonspecific interstitial pneumonia,NSIP)(25.64%)是较常见的类型,OP常与普通型间质性肺炎(usual interstitial pneumonia,UIP)或NSIP共存.明确病理诊断组的有效组织比例显著高于不确定诊断组(91.67%vs 55.56%,P<0.001).在CTD亚型中,以干燥综合征(Sjögren's syndrome,SS)和多发性肌炎/皮肌炎(polymyositis/dermatomyositis,PM/DM)为主,其病理表现与2025年ERS/ATS IP分类中的"重叠模式"概念相符.结论 TBLC能够获取高质量的肺组织,对CTD-ILD具有重要的诊断价值.所观察到的病理特征与2025年ERS/ATS IP分类更新内容相一致,支持TBLC联合多学科讨论在CTD-ILD诊断中的作用.
Objective To investigate the diagnostic value of transbronchial lung cryobiopsy(TBLC)in connec-tive tissue disease-associated interstitial lung disease(CTD-ILD)and analyze its pathological features based on the 2025 European Respiratory Society(ERS)/American Thoracic Society(ATS)interstitial pneumonia classification up-date.Methods A retrospective analysis included 195 patients with diffuse lung disease who underwent TBLC,of whom 29 were diagnosed with CTD-ILD via a comprehensive clinico-radiologic-pathologic(CRP)assessment.Data on patient demographics,clinical features,and TBLC pathological findings were collected.Results The 29 CTD-ILD patients accounted for 14.87%(29/195)of the TBLC cohort.The main clinical manifestations were cough(62.07%)and dyspnea(51.72%).Pathological patterns were heterogeneous and often mixed.Organizing pneumonia(OP,28.21%)and nonspecific interstitial pneumonia(NSIP,25.64%)were the most common patterns,with OP fre-quently coexisting with usual interstitial pneumonia(UIP)or NSIP.The effective tissue ratio was significantly higher in the definitive pathological diagnosis group compared to the suspicious diagnosis group(91.67%vs 55.56%,P<0.001).Among CTD subtypes,Sjögren's syndrome(SS)and polymyositis/dermatomyositis(PM/DM)were predomi-nant,and their pathological manifestations aligned with the"overlap pattern"concept of the 2025 ERS/ATS classifica-tion.Conclusion TBLC can yield high-quality lung tissue and holds significant diagnostic value for CTD-ILD.The observed pathological features are consistent with the 2025 ERS/ATS classification update,supporting the role of TBLC combined with multidisciplinary discussion in CTD-ILD diagnosis.
