间质性肺疾病病理诊断模式的更新与思考OA
Update and reflection on the pathological diagnostic paradigm of interstitial lung diseases
间质性肺疾病(interstitial lung disease,ILD)是一组高度异质性的非肿瘤性疾病,其诊断一直是临床和病理的难点.随着经支气管冷冻肺活检等微创技术的广泛应用、2025年间质性肺炎国际多学科分类的更新、以及人工智能(artificial intelligence,AI)等新技术的出现,ILD的病理诊断模式正经历深刻的变化.本文旨在剖析我国ILD病理诊断的现状与瓶颈,探讨新分类体系带来的启示,重申临床-影像-病理多学科诊断的基石地位,并展望新兴技术在提升诊断水平中的潜能.最终,提出整合性策略以应对当前困境,推动ILD诊断从依赖单一病理模式向动态、精准、整合的诊断与管理范式转变.
Interstitial lung disease(ILD)comprises a group of highly heterogeneous non-neoplastic disorders,and its pathological diagnosis has long remained a major challenge in both clinical practice and pathology.With the widespread application of minimally invasive techniques such as transbronchial cryobiopsy,the 2025 update of the in-ternational multidisciplinary classification of interstitial pneumonias,and the emergence of new technologies including artificial intelligence(AI),the pathological diagnostic paradigm for ILD is undergoing profound transformation.This article aims to analyze the current status and key bottlenecks of pathological diagnosis of ILD in China,discuss the in-sights provided by the updated classification system,reaffirm the cornerstone role of clinico-radiologic-pathologic mul-tidisciplinary diagnosis,and explore the potential of emerging technologies in improving diagnostic performance.Ulti-mately,an integrative strategy is proposed to address current challenges and to facilitate the transition of ILD diagnosis and management from reliance on a single pathological pattern toward a dynamic,precise,and integrated diagnostic and management paradigm.
