阶梯式运动康复对老年稳定期慢性阻塞性肺疾病患者呼吸力学及运动耐力的影响OA
Effect of stepwise exercise rehabilitation on respiratory mechanics and exercise endurance in elderly patients with stable chronic obstructive pulmonary disease
目的 探讨阶梯式运动康复对老年稳定期慢性阻塞性肺疾病(COPD)患者呼吸力学及运动耐力的影响.方法 选取2024年4-10月收治的80例老年稳定期COPD患者为研究对象,采用随机数字表法分为观察组和对照组,各40例.两组均行常规药物治疗,对照组采用常规运动康复训练,观察组采用阶梯式运动康复训练,两组均连续训练8周.对比两组肺功能指标[第1秒用力呼气容积占预估值百分比(FEV1%)、用力肺活量(FVC)、最大呼气流速峰值(PEF)]、呼吸力学指标[呼吸频率、潮气量、呼吸道压峰值(Ppeak)]、静态与运动血氧指标[动脉血氧饱和度(SaO2)、氧减指数、低氧比率]、生化指标[外周血降钙素原(PCT)、血小板/淋巴细胞比值(PLR)、白细胞介素-6(IL-6)、中性粒细胞/淋巴细胞比(NLR)]、运动耐力[6 min步行距离(6MWD)]、睡眠质量[匹兹堡睡眠质量量表(PSQI)]、生活质量[圣乔治医院呼吸问题调查问卷(SGRQ)]、急性加重事件发生率.结果 训练后,观察组呼吸频率、Ppeak低于对照组,FVC、潮气量、FEV1%、PEF高于对照组(P<0.01).训练后,观察组氧减指数、低氧比率低于对照组(P<0.0.1),两组SaO2比较差异无统计学意义(P>0.05).训练后,观察组外周血PCT、PLR、IL-6、NLR水平低于对照组(P<0.01).训练后,观察组PSQI、SGRQ评分低于对照组,6MWD高于对照组(P<0.01).观察组康复训练期间急性加重事件发生率[2.50%(1/40)]较对照组[20.00%(8/40)]低(P<0.05).结论 阶梯式运动康复应用于老年稳定期COPD患者中可改善患者运动时血氧状态,降低低机体炎症水平,调整呼吸力学,改善肺功能,提升运动耐力,从而提高患者生活质量与睡眠质量,临床应用安全性高.
Objective To explore the impact of stepwise exercise rehabilitation on respiratory mechanics and exercise tolerance in elderly patients with stable chronic obstructive pulmonary disease(COPD).Methods A total of 80 elderly patients with stable COPD admitted from April to October 2024 were selected as the study subjects.The patients were randomly divided into an observation group and a control group using a random number table method,with 40 patients in each group.Both groups received conventional drug therapy,while the control group received conventional exercise rehabilitation training,and the observation group received stepwise exercise rehabilitation training.Both groups underwent continuous training for 8 weeks.The pulmonary function indicators[percentage of predicted forced expiratory volume in 1 second(FEV1%predicted),forced vital capacity(FVC),peak expiratory flow rate(PEF)],respiratory mechanics indicators[respiratory rate,tidal volume,peak airway pressure(Ppeak)],static and exercise blood oxygen indexes[arterial oxygen saturation(SaO2),oxygen desaturation index(ODI),hypoxemia ratio],biochemical indicators[peripheral blood procalcitonin(PCT),platelet/lymphocyte ratio(PLR),interleukin-6(IL-6),neutrophil/lymphocyte ratio(NLR)],exercise tolerance[6-minute walking distance(6MWD)],sleep quality[Pittsburgh Sleep Quality Index(PSQI)],quality of life[St.George's Hospital Respiratory Questionnaire(SGRQ)],and incidence of acute exacerbation events were compared between the two groups.Results After training,the observation group had lower respiratory rate and Ppeak,and higher FVC,tidal volume,FEV1%,and PEF compared with the control group(P<0.01).After training,the ODI and hypoxia ratio in the observation group were lower than those in the control group(P<0.01),and no significant difference in SaO2 was found between the two groups(P>0.05).After training,the levels of peripheral blood PCT,PLR,IL-6 and NLR in the observation group were lower than those in the control group(P<0.01).The observation group presented lower PSQI,SGRQ scores and higher 6MWD than the control group after training(P<0.01).The incidence of acute exacerbation events during rehabilitation training in the observation group was[2.50%(1/40)],which was lower than[20.00%(8/40)]in the control group(P<0.05).Conclusion The application of stepwise exercise rehabilitation in elderly stable COPD patients can improve that blood oxygen state of patient during exercise,reduce the level of inflammation,adjust respiratory mechanics,improve pulmonary function,enhance exercise tolerance,and thus improve patients' quality of life and sleep quality,with good safety for clinical application.
