目的 探讨脑脊液(CSF)基质金属蛋白酶(MMPs)水平与阿尔茨海默病(Alzheimer's disease,AD)核心病理及认知功能的关系.方法 选择阿尔茨海默病神经影像倡议(ADNI)数据库中受试者497例,收集其CSF MMPs(MMP-2、MMP-3、MMP-10 及 MMP-9/TIMP-1 比值)水平、AD 核心病理(Aβ42、P-tau、T-tau)水平及AD认知功能相关评分(MMSE、ADNI_MEM、ADNI_EF、ADNI_LAN及ADNI_VS评分)结果.采用多元线性回归评估CSF MMPs水平与 CSF AD核心病理水平及AD认知功能是否存在线性关联,采用线性混合效应模型探讨CSF MMPs水平对AD认知功能下降的影响,通过中介分析评估CSF AD核心病理水平对CSF MMPs水平与 AD认知功能关系的中介作用.结果 多元线性回归分析结果显示,CSF MMP-10、MMP-3及MMP-9/TIMP-1比值水平均与CSF P-tau、T-tau水平呈显著正相关(P<0.05),CSF MMP-10与 CSF Aβ42水平呈负相关(P<0.05);CSF MMP-10水平与 ADNI_MEM、ADNI_EF、ADNI_LAN及ADNI_VS得分均呈显著负相关(P<0.05).线性混合效应模型结果显示,CSF MMP-10高水平与 ADNI_MEM、ADNI_EF、ADNI_LAN、ADNI_VS等认知功能相关评分快速下降有关(P<0.05).中介分析结果显示,CSF Aβ42、P-tau、T-tau介导了 CSF MMP-10与认知功能间的关系.结论 CSF MMP-10水平与 CSF AD核心病理水平及认知功能衰退均表现出显著关联,CSF MMP-10水平可能通过影响AD核心病理参与认知下降的过程.

Objective To investigate the association of the levels of matrix metalloproteinases(MMPs)in cerebrospinal fluid(CSF)with the core pathological biomarkers of Alzheimer's disease(AD)and the cognitive function of AD.Methods A total of 497 subjects were selected from the Alzheimer's Disease Neuroimaging Initiative(ADNI)database,and related data were collected,including the levels of CSF MMPs(MMP-2,MMP-3,MMP-10,and MMP-9/TIMP-1 ratio),the core biomarkers for AD(Aβ42,phosphorylated tau[P-tau],and total tau[T-tau]),and cognitive function scores(MMSE,ADNI_MEM,ADNI_EF,ADNI_LAN,and ADNI_VS).The multiple linear regression analysis was used to assess whether the levels of CSF MMPs had a linear association with the core pathological biomarkers of AD and the cognitive function of AD.The linear mixed-effects model was used to evaluate the effect of the levels of CSF MMPs on cognitive decline.A mediation analysis was used to assess the media-ting effect of the core pathological biomarkers of AD in the association between CSF MMPs and cognitive function.Results The multiple linear regression analysis showed that CSF MMP-10,MMP-3,and MMP-9/TIMP-1 ratio were positively associated with the levels of CSF P-tau and T-tau(P<0.05),while CSF MMP-10 was negatively associated with CSF Aβ42(P<0.05),and the level of CSF MMP-10 was negatively associated with ADNI_MEM,ADNI_EF,ADNI_LAN,and ADNI_VS scores(P<0.05).The linear mixed-effects model analysis showed that the high level of CSF MMP-10 was associated with the rapid reductions in cog-nitive function scores such as ADNI_MEM,ADNI_EF,ADNI_LAN,and ADNI_VS(P<0.05).The mediation analysis showed that CSF Aβ42,P-tau,and T-tau mediated the association between CSF MMP-10 and cognitive function.Conclusion CSF MMP-10 level is significantly associated with the core pathological biomarkers of AD and cognitive decline in AD,and CSF MMP-10 may be involved in cognitive decline by affecting the core pathological biomarkers of AD.
