高强度间歇训练理念有氧舞蹈对老年轻度认知障碍执行功能的影响及机制OA
Effects of aerobic dance based on high-intensity interval training concept on executive function in elderly patients with mild cognitive impairment and its mechanisms
目的 观察基于高强度间歇训练(HIIT)理念的有氧舞蹈运动对老年轻度认知障碍(MCI)患者认知执行功能的改善作用,并通过对比干预前后患者血浆谷氨酸、精氨酸和丙二醛(MDA)水平的变化,从"兴奋性毒性-氧化应激"角度探讨其作用机制.方法 本研究为一项多中心前瞻性、随机、对照临床试验,以2021年3月—2022年3月在江苏省苏北人民医院康复医学科与扬州市中医院脑病科共同招募的老年MCI患者为研究对象.干预3个月后,45例受试者最终纳入本研究,其中运动组23例,对照组22例.运动组接受基于HIIT理念的有氧舞蹈干预,持续干预3个月,对照组仅接受相关健康教育.结果 干预后,运动组的蒙特利尔认知评估(MoCA)评分、数字广度测试(DST)评分、连线测试B部分(TMT-B)、Stroop色词测试干扰效应(SIE)耗时、最大摄氧量(VO2max)、血浆精氨酸、谷氨酸和MDA指标均较干预前有所改善,差异有统计学意义(P<0.05).干预后,运动组执行功能量表DST评分、TMT-B、SIE评分、最大摄氧量、血浆谷氨酸和MDA指标均优于对照组,差异有统计学意义(P<0.05).结论 基于HIIT理念设计的有氧舞蹈运动干预,能显著改善老年MCI患者的执行功能,其作用机制可能与血浆谷氨酸和精氨酸水平的降低以及脑内氧化应激损伤的减轻有关,其所涉及的"兴奋性毒性-氧化应激"调控代谢通路为理解运动益脑的神经生物学机制以及后续相关生物标志物研究提供了新视角.
Objective To observe the improvement effect of aerobic dance based on the high-in-tensity interval training(HIIT)concept on cognitive executive function in elderly patients with mild cogni-tive impairment(MCI)and explore its underlying mechanisms from the perspective of"excitotoxicity-oxi-dative stress"by comparing the changes in plasma glutamate,arginine,and malondialdehyde(MDA)lev-els before and after intervention.Methods This was a multi-center,prospective,randomized,controlled clinical trial.Elderly patients with MCI recruited jointly by the Department of Rehabilitation Medicine of the Northern Jiangsu People's Hospital of Jiangsu Province and the Department of Encephalopathy of the Yangzhou Traditional Chinese Medicine Hospital from March 2021 to March 2022 were includ-ed as the study subjects.After 3 months of intervention,45 subjects were finally included in this study,with 23 in the exercise group and 22 in the control group.The exercise group received aero-bic dance intervention based on the HIIT concept for 3 months,while the control group only received relevant health education.Results After intervention,the Montreal Cognitive Assessment(MoCA)score,Digital Span Test(DST),Trail Making Test Part B(TMT-B),Stroop Color-Word Test In-terference Effect(SIE)time-consuming,maximal oxygen uptake(VO2max),and plasma arginine,glutamate,and MDA levels in the exercise group showed significant improvements compared with those before intervention(P<0.05).After intervention,The DST score,TMT-B,SIE score,VO2max,and plasma glutamate and MDA levels in the exercise group were superior to those in the control group,with statistically significant differences(P<0.05).Conclusion Aerobic dance in-tervention designed based on the HIIT concept can significantly improve executive function in elderly patients with MCI.Its underlying mechanisms may be related to the reduction in plasma glutamate and arginine levels and the alleviation of brain oxidative stress injury.The"excitotoxicity-oxidative stress"regulatory metabolic pathway provides a new perspective for understanding the neurobiologi-cal mechanisms of exercise benefits for the brain and subsequent related biomarker research.
