Association between high—sensitivity C—reactive protein and cognitive function in patients with schizophrenia:A cross—sectional study using the MCCB

目的:采用精神分裂症认知功能成套测验—共识版(MATRICS Consensus Cognitive Battery,MCCB)评估精神分裂症患者的多域认知功能,探讨血清超敏C反应蛋白(hs-CRP)水平与认知功能的关联及性别差异.方法:纳入2024年10月至2025年7月某精神卫生中心住院、处于稳定期的精神分裂症患者160例,采用MCCB评估7个认知域功能,化学发光法测定hs-CRP水平,阳性与阴性症状量表(PANSS)评估精神症状,收集人口学与临床资料.通过独立样本t检验、Mann-Whitney U检验、x2检验、Spearman相关分析及多元线性回归(调整年龄、性别、受教育年限、BMI、病程、PANSS得分及抗精神病药物剂量)分析hs-CRP与认知功能的关系,并进行性别分层及交互效应检验.结果:160例患者中hs-CRP高水平组(≥3 mg/L)51例(31.9%),低水平组(<3 mg/L)109例(68.1%),两组BMI差异有统计学意义(t=-0.664,P=0.039).Spearman相关显示,hs-CRP水平与MCCB总分及信息处理速度、注意/警觉性、工作记忆、视觉学习、社会认知域得分呈显著负相关(r=-0.157~-0.252,P<0.05).多元线性回归(调整协变量后)显示,ln-hsCRP与MCCB总分(β=-0.214,P=0.003)、视觉学习(β=-0.231,P=0.004)及社会认知(β=-0.235,P=0.005)呈独立负相关.性别分层分析显示,男性患者中ln-hsCRP与MCCB总分(β=-0.279,P=0.021)、视觉学习(β=-0.304,P=0.013)负相关,女性患者中ln-hsCRP与社会认知(β=-0.244,P=0.029)负相关,但性别× ln-hsCRP交互效应无统计学意义(P>0.05).结论:精神分裂症患者血清hs-CRP水平升高与认知功能损害显著相关,尤其在视觉学习和社会认知领域表现突出.不同性别患者呈现差异化模式:男性更易受影响于总体认知和视觉学习,女性则主要涉及社会认知,但交互作用未达显著.结果提示炎症可能是精神分裂症认知障碍的重要生物学机制,具有潜在的临床评估与干预价值.

Objective:To evaluate multidomain cognitive function in patients with schizophrenia using the MAT-RICS Consensus Cognitive Battery(MCCB)and to explore the association between serum high-sensitivity C-reac-tive protein(hs-CRP)levels and cognition function as well as gender differences.Methods:A total of 160 clinically stable inpatients with schizophrenia were consecutively recruited from October 2024 to July 2025 at the Mental Health Center,Zhejiang University School of Medicine.Cognitive function was assessed with the MCCB,serum hs-CRP was measured by chemiluminescence,and psychiatric symptoms were evaluated with the Positive and Negative Syndrome Scale(PANSS).Demographic and clinical variables were collected.Independent-sample t-tests,Mann-Whitney U test,x2 test,Spearman correlations,and multivariable linear regression models(adjusting for age,sex,years of education,body mass index[BMI],illness duration,PANSS scores,and antipsychotic dose)were used to ex-amine the relationships between hs-CRP and cognition,as well as gender stratification and interaction tests were conducted.Results:Among 160 participants,51(31.9%)were in the high hs-CRP group(≥3 mg/L)and 109(68.1%)in the low group(<3 mg/L).BMI differed significantly between groups(t=-0.664,P=0.039).Spearman analyses showed significant negative correlations between hs-CRP and the MCCB composite score as well as pro-cessing speed,attention/vigilance,working memory,visual learning,and social cognition(r=-0.157 to-0.252;P<0.05).After adjustment,ln-hsCRP remained independently associated with the MCCB composite(β=-0.214,P=0.003),visual learning(β=-0.231,P=0.004),and social cognition(β=-0.235,P=0.005).In sex-stratified analy-ses,ln-hsCRP was negatively associated with the MCCB composite(β=-0.279,P=0.021)and visual learning(β=-0.304,P=0.013)in males,and with social cognition(β=-0.244,P=0.029)in females;however,no significant sex × hs-CRP interaction effects were observed(P>0.05).Conclusion:Elevated serum hs-CRP levels are significantly associated with cognitive impairment in patients with schizophrenia,particularly in visual learning and social cogni-tion.Different patterns are observed between sexes,with males more affect in overall cognition and visual learning,and females mainly in social cognition,although no significant interaction is detected.These findings suggest that in-flammation may represent an important biological mechanism underlying cognitive deficits in schizophrenia and may serve as a potential target for clinical assessment and intervention.
