基于逻辑回归模型的青少年高血压风险预测研究OA
Study on risk prediction of adolescent hypertension based on logistic regression model
本研究旨在探究体检指标对青少年高血压风险预测.以 2020-2024年在江西省某高校进行了入学体检管理的学生为研究对象(n=27 490),根据青少年高血压诊断标准分为三组,采用 Kruskal-Wallis 检验分析基线特征.为评估独立关联性,利用逻辑回归分析体质指数及相关指标与高血压状态的关系.采用限制性立方样条(RCS)检验身体质量指数(BMI)及相关指标与高血压风险之间的剂量反应关系.最后运用受试者工作特征(ROC)曲线分析量化各指标对高血压及高血压前期的预测效能.与正常血压组相比,高血压组的BMI、脉搏、生化指标中位数与 BMI相关参数均显著升高(P<0.001).高血压前期组的指标水平介于正常血压组与高血压组之间.逻辑回归分析显示,BMI 及相关指标是青少年高血压的独立危险因素.RCS 分析表明 BMI 及相关指标与青少年高血压风险呈现多形态的非线性关系(P<0.001).BMI 指标对高血压(AUC=0.71,95%CI=0.68~0.73)和高血压前期(AUC=0.66,95%CI=0.65~0.67)的诊断效能最高,其最佳截断值分别为24.29和23.88.BMI 对青少年高血压及其高血压前期展现出较好的鉴别性能,并呈现与高血压风险之间反"N"型关系.
This study aims to explore the predictive value of physical examination indicators for hypertension risk in adolescents.A total of 27,490 students underwent with physical examination at a university in Jiangxi Province from 2020 to 2024 were recruited in this study.Based on the diagnostic criteria for adolescent hypertension,the participants were stratified into three groups,and the Kruskal-Wallis test was used to analyze the baseline characteristics of each group.To evaluate independent associations,logistic regression was used to explore the relationship between body mass index(BMI),its related indicators and hypertension risk.Restricted cubic splines(RCS)were employed to investigate the dose-response relationship between BMI,its related indicators and hypertension risk.Additionally,receiver operating characteristic(ROC)curve analysis was performed to quantify the predictive ability of each indicator for hypertension and prehypertension.Compared with the normotensive group,the hypertension group had significantly higher median values of BMI,pulse rate,biochemical indicators,and BMI-related parameters(P<0.001).The prehypertension group demonstrated intermediate levels of all measured indices between the normotensive and hypertensive groups.Logistic regression analyses demonstrated that BMI and its related indicators were independent risk factors for adolescent hypertension.RCS analyses confirmed polymorphic non-linear relationships between BMI and adolescent hypertension risk in s(P<0.001).For hypertension and prehypertension,BMI achieved the highest area under the curve(AUC)of 0.71(95%CI=0.68-0.73)and 0.66(95%CI=0.65-0.67),respectively,with corresponding optimal cutoffs of 24.29 and 23.88.BMI demonstrated favorable discriminative ability for both adolescent hypertension and prehypertension,and an inverted"N"-shaped relationship was observed between BMI and hypertension risk.
