Effects of Huangzhi Lishang Plaster on NF-κB signaling pathway in cartilage tissue of rabbit knee osteoarthritis

目的 探讨黄栀理伤贴膏对兔膝骨关节炎(KOA)软骨组织核因子κB(NF-κB)信号通路的影响.方法 将24只新西兰兔随机分为空白组(6只)和造模组(18只).造模组采用膝关节腔内注射木瓜蛋白酶与L-半胱氨酸混合液的方式建立KOA模型,空白组注射等量生理盐水.4周后验证模型,将造模成功的兔随机分为模型组(胶布敷贴)、骨通贴膏组(骨通贴膏敷贴)和黄栀理伤贴膏组(黄栀理伤贴膏敷贴),每组5只.干预均为1次/d,每次8h,持续4周.观察各组兔膝关节一般情况并进行Lequesne MG评分评估骨关节炎严重程度,比较膝关节影像学表现、膝关节软骨大体观察,采用Pelletier评分评估软骨组织损伤情况,ELISA检测血清白细胞介素(IL)-1β含量,Western blot检测软骨组织NF-κB、NF-κB p50蛋白表达水平.结果 通过对比各组膝关节X线,模型组发生不同程度的退行性改变,骨通贴膏组、黄栀理伤贴膏组较模型组减轻.软骨大体观察结果显示,模型组膝关节结构改变、骨赘形成、软骨损伤,骨通贴膏组、黄栀理伤贴膏组膝关节较模型组改善.与空白组比较,模型组Lequesne MG评分、Pelletier评分、血清IL-1β含量及NF-κB、NF-κB p50蛋白表达均升高(P<0.001);与模型组比较,骨通贴膏组及黄栀理伤贴膏组Lequesne MG评分、Pelletier评分、血清IL-1β含量及NF-κB、NF-κB p50蛋白表达均降低(P<0.001);与骨通贴膏组比较,黄栀理伤贴膏组血清IL-1β含量及NF-κB、NF-κB p50蛋白表达均降低(P<0.001).结论 黄栀理伤贴膏可能通过抑制NF-κB信号通路、降低IL-1β炎症因子表达,从而缓解KOA软骨损伤.

Objective To investigate the effects of Huangzhi Lishang Plaster on nuclear factor-κB(NF-κB)signaling pathway in cartilage tissue of rabbits with knee osteoarthritis(KOA).Methods A total of 24 New Zealand rabbits were randomly divided into a blank control group(n=6)and a model establishment group(n=18).The KOA model was established by intra-articular injection of a mixture of papain and L-cysteine in the model group,while the blank group received an equal volume of normal saline.The modeling was verified 4 weeks later.Successfully modeled rabbits were randomly divided into a model group(treated with adhesive plaster application),a Gutong Plaster group(treated with Gu tong Plaster),and a Huangzhi Lishang Plaster group(treated with Huangzhi Lishang Plaster),with five rabbits in each group.The interventions were applied once daily for 8 hours each time,lasting for four consecutive weeks.The general condition of the knee joint was observed,and the Lequesne MG score was used to evaluate the severity of osteoarthritis.The imaging findings of the knee joint and gross observation of knee cartilage were compared.The Pelletier score was used to evaluate the cartilage tissue damage.Serum interleukin-1β(IL-1β)level was measured by ELISA,and the protein expression levels of NF-κB and NF-κB p50 in cartilage tissue were checked by Western blot.Results Comparison of knee joint X-rays among groups showed that varying degrees of degenerative changes were present in the model group,which were alleviated in the Gutong Plaster group and Huangzhi Lishang Plaster group compared with the model group.Gross observation of cartilage indicated structural changes,osteophyte formation,and cartilage damage in the model group,with improvements observed in both treatment groups.Compared with the blank control group,the model group had significantly higher Lequesne MG score,Pelletier score,serum IL-1β level,and protein expression levels of NF-κB and NF-κB p50(P<0.001).Compared with the model group,the above indexes were all significantly decreased in the Gutong Plaster group and Huangzhi Lishang Plaster groups(P<0.001).Compared with the Gutong Plaster group,the Huangzhi Lishang Plaster group exhibited further reductions in serum IL-1β level and protein expressions of NF-κB and NF-κB p50(P<0.001).Conclusion Huangzhi Lishang Plaster may alleviate cartilage damage in KOA by inhibiting the NF-κB signaling pathway and reducing the expression of inflammatory factor IL-1β.
