目的 分析龙岩市中医院 2022 年至 2024 年临床常见病原菌分布、耐药特征及科室聚集情况,为精准用药与感染防控提供依据.方法 2022 年 1月至 2024 年12 月期间收集龙岩市中医院 2072 份临床微生物标本,经标准化分离培养、DL-96A系统鉴定菌种,参照CLSI 标准行药敏试验,采用 SPSS 26.0 统计分析数据.结果 分离出的 2072 株病原菌中革兰氏阴性菌占87.98%,铜绿假单胞菌、大肠埃希菌、肺炎克雷伯菌为优势菌;革兰阳性菌占 12.02%,以金黄色葡萄球菌为主.鲍曼不动杆菌检出占比逐年升至 8.62%,产气克雷伯菌降至 2.44%.耐药性上,铜绿假单胞菌对阿米卡星耐药率最低、左氧氟沙星最高;鲍曼不动杆菌对所有检测药物高耐药;金黄色葡萄球菌对阿米卡星耐药率最低、阿莫西林/克拉维酸最高.科室分布中,铜绿假单胞菌、肺炎克雷伯菌聚集于康复二科与重症医学科,鲍曼不动杆菌集中于重症医学科.结论 医院临床感染以革兰氏阴性菌为主,鲍曼不动杆菌占比上升及多重耐药问题突出,需结合病原菌特征优化抗菌药物使用,强化重点科室感染防控.

Objective To analyze the distribution,drug resistance characteristics,and departmental clustering of common clinical pathogens in Longyan Traditional Chinese Medicine Hospital from 2022 to 2024,providing a basis for precision medication and infection prevention and control.Method From January 2022 to December 2024,2072 clinical microbiological specimens were collected from Longyan Traditional Chinese Medicine Hospital.The strains were identified by standardized isolation and culture,DL-96A system,and drug sensitivity tests were conducted according to CLSI standards.SPSS 26.0 was used for statistical analysis of the data.Result Among the 2072 isolated pathogenic bacteria,Gram negative bacteria accounted for 87.98%,with Pseudomonas aeruginosa,Escherichia coli,and Klebsiella pneumoniae being the dominant bacteria;Gram positive bacteria account for 12.02%,mainly Staphylococcus aureus.The detection rate of Acinetobacter baumannii has increased year by year to 8.62%,while that of Klebsiella pneumoniae has decreased to 2.44%.In terms of drug resistance,Pseudomonas aeruginosa has the lowest resistance rate to amikacin and the highest resistance rate to levofloxacin;Acinetobacter baumannii is highly resistant to all tested drugs;Staphylococcus aureus has the lowest resistance rate to amikacin and the highest resistance rate to amoxicillin/clavulanic acid.In the distribution of departments,Pseudomonas aeruginosa and Klebsiella pneumoniae are concentrated in the Rehabilitation Department and Intensive Care Medicine Department,while Acinetobacter baumannii is concentrated in the Intensive Care Medicine Department.Conclusion Clinical infections in hospitals are mainly caused by gram-negative bacteria,with an increasing proportion of Acinetobacter baumannii and prominent issues of multidrug resistance.It is necessary to optimize the use of antibiotics based on the characteristics of pathogenic bacteria and strengthen infection prevention and control in key departments.
