加味葛根芩连汤治疗晚期大肠癌临床研究OA

Clinical study of modified Gegen Qinlian Decoction in the treatment of advanced colorectal cancer

目的 观察加味葛根芩连汤治疗晚期大肠癌的临床效果.方法 选择 2022 年 12 月至 2023 年 12 月唐山市人民医院中西医结合肿瘤科晚期大肠癌患者 118 例,采用随机数字表法分为 2 组.对照组 59 例,采用常规 XELOX 化疗;治疗组 59 例,在对照组基础上联合加味葛根芩连汤治疗.比较 2 组治疗前后肠道菌群含量、血清肿瘤相关因子水平、T 淋巴细胞比例及炎症因子水平.统计并比较 2 组临床疗效和不良反应.结果 治疗后,2 组双歧杆菌、乳酸杆菌含量均高于本组治疗前(P<0.05),肠球菌、肠杆菌含量均低于本组治疗前(P<0.05),且治疗后治疗组双歧杆菌、乳酸杆菌含量均高于对照组(P<0.05),肠球菌、肠杆菌含量均低于对照组(P<0.05).治疗后,2 组血管内皮细胞生长因子、基质金属蛋白酶 2、基质金属蛋白酶 7、基质金属蛋白酶 9、超敏 C 反应蛋白、肿瘤坏死因子 α、白细胞介素 6 水平均降低(P<0.05),且治疗组均低于对照组(P<0.05).治疗后,2 组 CD8+均高于本组治疗前(P<0.05),CD4+、CD4+/CD8+均低于本组治疗前(P<0.05),但治疗后治疗组 CD4+、CD4+/CD8+均高于对照组(P<0.05),CD8+低于对照组(P<0.05).治疗组总有效率为98.31%(58/59),对照组总有效率为86.44%(51/59),治疗组总有效率显著高于对照组(P<0.05).治疗组不良反应总发生率为 1.69%(1/59),对照组为 15.25%(9/59),治疗组不良反应总发生率低于对照组(P<0.05).结论 加味葛根芩连汤治疗晚期大肠的临床疗效显著,可改善患者肠道菌群,减轻机体炎症反应,降低血清肿瘤相关因子水平,提高免疫力,安全性高.

Objective To observe the clinical efficacy of modified Gegen Qinlian Decoction in the treatment of advanced colorectal cancer.Methods A total of 118 patients with advanced colorectal cancer admitted to the Department of Oncology of Integrated Traditional Chinese and Western Medicine,Tangshan People's Hospital,from December 2022 to December 2023 were selected and randomly divided into two groups using a random number table method.The control group included 59 patients and received conventional XELOX chemotherapy.The treatment group included 59 patients and received modified Gegen Qinlian De-coction in addition to the treatment given to the control group.Intestinal microbiota levels,serum tumor-related factor levels,T-lymphocyte proportions,and inflammatory factor levels were compared between the two groups before and after treatment.Clinical efficacy and adverse reactions were also recorded and compared.Results After treatment,both groups showed increased levels of Bifidobacterium and Lactobacillus compared with their pre-treatment levels(P<0.05),while Enterococcus and Enterobacter levels decreased compared with their pre-treatment levels(P<0.05).After treatment,the treatment group had higher levels of Bifidobacterium and Lactobacillus than the control group(P<0.05),and lower levels of Enterococcus and Enterobacter than the control group(P<0.05).After treatment,the levels of vascular endothelial growth factor,matrix metalloproteinase-2,matrix metalloproteinase-7,matrix metalloproteinase-9,hypersensitive C-reactive protein,tumor necrosis factor-α,and interleukin-6 decreased in both groups(P<0.05),with all levels being lower in the treatment group than in the control group(P<0.05).After treatment,CD8+levels increased in both groups compared with pre-treatment levels(P<0.05),whereas CD4+and CD4+/CD8+ratios decreased in both groups compared with pre-treatment levels(P<0.05).However,after treatment,the treatment group had higher CD4+and CD4+/CD8+ratios than the control group(P<0.05),and lower CD8+levels than the control group(P<0.05).The total effective rate was 98.31%(58/59)in the treatment group and 86.44%(51/59)in the control group,with the treatment group showing a significantly higher total effective rate than the control group(P<0.05).The overall incidence of adverse reactions was 1.69%(1/59)in the treatment group and 15.25%(9/59)in the control group,with the treatment group showing a lower incidence than the control group(P<0.05).Conclusion Modified Gegen Qinlian Decoction shows a significant clinical effect in the treatment of advanced colorectal cancer.It can im-prove intestinal microbiota,alleviate systemic inflammatory responses,reduce serum tumor-related factor levels,enhance immu-nity,and has good safety.
