皮肤牵张器在大面积创面修复中的应用现状OA
Current application of skin stretchers in large area wound repair
皮肤牵张技术是修复大面积创面的理想策略.本文通过检索与分析相关临床研究文献,系统归纳了皮肤牵张器的临床应用情况.根据皮肤锚定方式与力学传递路径,目前用于大面积创面的皮肤牵张器可分为三类:轴锚式对向牵张器、线缆式连续牵张器与多束并行牵张器.轴锚式对向牵张器(以Sure-Closure为代表)牵拉力度强、适用范围广,但存在穿刺点皮肤缺血性坏死的风险,且难以适应不规则创面.线缆式连续牵张器(以DermaClose为代表)力线分布分散,对锚点皮肤的损伤小,并能灵活顺应不规则创面,但总体牵拉力相对有限.多束并行牵张器(以TopClosure为代表)在维持总体拉力的同时实现应力均匀分散,有效降低局部压强及相关并发症风险,同时兼具良好的创面形状适应性.尽管上述皮肤牵张器为大面积创面的闭合提供了行之有效的解决方案,但目前仍缺乏针对特定创面类型的大规模、多中心高质量临床研究.未来需进一步开展循证等级更高的研究,以明确各类技术的优势人群与最佳应用场景,推动皮肤牵张技术向更规范化、精准化的方向发展.
Skin stretching technology represents an ideal strategy for the repair of large area wounds.By retrieving and analyzing relevant clinical studies,this article systematically summarizes the clinical application status of skin stretchers.Based on skin anchoring methods and mechanical force transmission pathways,skin stretchers currently used for large area wounds can be classified into three types:axle-anchored counter-traction stretchers,cable-type continuous traction stretchers,and multi-bundle parallel traction stretchers.Axle-anchored counter-traction stretchers(represented by Sure-Closure)provide strong traction force and have a wide range of indications;however,they carry the risk of ischemic necrosis of the skin at the puncture sites and are poorly adaptable to irregular wounds.Cable-type continuous traction stretchers(represented by DermaClose)distribute the force lines more dispersedly,cause less damage to the anchor site skin,and can flexibly conform to irregular wound contours;but their overall traction force is relatively limited.Multi-bundle parallel traction stretchers(represented by TopClosure)achieve uniform stress distribution while maintaining overall traction force,effectively reducing local pressure and the risk of associated complications,while also offering good adaptability to wound shape.Although the aforementioned skin stretchers provide effective solutions for the closure of large area wounds,there remains a lack of large-scale,multicenter,high-quality clinical studies targeting specific wound types.Future research with higher levels of evidence is needed to clarify the optimal patient populations and best application scenarios for each technology,thereby promoting the development of skin stretching technology toward greater standardization and precision.
