肺部超声在新生儿呼吸窘迫综合征中的诊断价值及应用OA
The diagnostic value and application of lung ultrasound in neonatal respiratory distress syndrome
肺部超声(Lung ultrasound,LUS)是一种无电离辐射的常用医学影像技术,多项研究已证实对新生儿呼吸窘迫综合征(Neonatal respiratory distress syndrome,NRDS)具有良好的诊断敏感性与特异性.基于LUS B线评分及肺实变范围建立的LUS评分系统,可对NRDS患儿病情实施动态监测,并预测呼吸机相关性肺炎、支气管肺发育不良等不良并发症的发生风险,从而为临床决策提供重要依据.但含气肺泡、肋骨及肩胛骨遮挡使其深部病灶检出受限,导致部分病灶漏诊及LUS应用受限.随着人工智能(Artificial intelligence,AI)技术的快速发展,AI辅助诊断系统与LUS技术的融合应用有望突破传统LUS的物理局限,将显著提升NRDS诊断的准确性和时效性.本文对LUS技术在NRDS诊断、病情评估、指导临床用药、预测不良并发症的研究进展进行综述.
Lung ultrasound(LUS)is a non-ionizing radiation medical imaging technique widely used in clinical practice.The diagnostic sensitivity and specificity in neonatal respiratory distress syndrome(NRDS)have already been cofirmed by multiple studies.The scoring system based on B-line scores and the extent of pulmonary consolidation enables NRDS patients to be observed on demand to predicts the risk of adverse complications such as ventilator-associated pneumonia and bronchopulmonary dysplasia(BPD),providing important references for clinical decision-making.However,the detection of deep-seated lesions is limited by air-filled alveoli,rib shadows,and scapular obstruction,potentially leading to missed diagnoses and restricted clinical application.With the rapid advancement of artificial intelligence technology,the integration of AI-assisted diagnostic systems with LUS holds promise for overcoming the physical limitations of traditional LUS,significantly enhancing the accuracy and timeliness of NRDS diagnosis.This paper reviews the progress in lung ultrasound technology for the diagnosis of NRDS,disease severity assessment,guidance of pulmonary surfactant therapy,and prediction of adverse complications.
