前交叉韧带重建术后髋、膝、踝关节周围肌群在不同角速度下的等速肌力特征研究OA
Isokinetic muscle strength characteristics of hip,knee,and ankle muscles at different angular velocities following anterior cruciate ligament reconstruction
目的 评估前交叉韧带重建(anterior cruciate ligament reconstruction,ACLR)术后患者下肢髋、膝、踝关节周围肌群的肌肉力量,并分析不同关节及运动速度下的肌力失衡规律,为优化术后康复策略提供理论依据.方法 采用等速肌力测试系统,选取40例ACLR术后患者与20例健康对照组进行比较,测试其髋、膝、踝关节周围肌群在不同角速度下的等速肌力,并获取峰力矩等关键指标数据,分析患者与健康对照组、患侧之间的肌力差异.结果 在60°/s,120°/s角速度下,健康组和ACLR组髋关节外展、内收肌群相对峰力矩(relative peak torque,RPT)及外展/内收比值,差异有统计学意义(P<0.05).健康组和ACLR组患者膝关节伸展、屈曲肌群RPT及屈曲/伸展比值,差异有统计学意义(P<0.05).健康组和ACLR组的踝关节内翻、外翻、跖屈及背伸肌群RPT,差异有统计学意义(P<0.05).结论 ACLR术后患者下肢髋、膝、踝关节周围肌群等速肌力存在显著下降与失衡,且不同关节、不同运动速度下的肌力变化具有特异性.为促进患者术后肌力恢复与运动功能改善,临床康复治疗需依据各关节肌群肌力特征制定个性化方案.
Objective To evaluate isokinetic muscle strength of the hip,knee,and ankle musculature in patients following anterior cruciate ligament reconstruction(ACLR),and to analyze muscle strength imbalances across different joints and angular velocities,thereby providing evidence for optimizing postoperative rehabilitation strategies.Methods An isokinetic dynamometer was used to assess muscle strength in 40 patients after ACLR and 20 healthy controls.Isokinet-ic testing of muscle groups surrounding the hip,knee,and ankle joints was performed at different angular velocities.Key parameters,including relative peak torque(RPT)and muscle strength ratios,were recorded.Differences between ACLR patients and healthy controls,as well as between limbs,were analyzed.Results At angular velocities of 60°/s and 120°/s,significant differences were observed between the ACLR group and the control group in hip abductor and adductor RPT values and abductor/adductor ratios(P<0.05).For the knee joint,significant differences were found in extensor and flexor RPT values as well as flexor/extensor ratios between the two groups(P<0.05).Additionally,significant differences were identified in ankle invertor,evert or,plantarflexor,and dorsiflexor RPT values between groups(P<0.05).Conclusion Patients following ACLR exhibit significant reductions and imbalances in isokinetic muscle strength of the hip,knee,and ankle musculature.These alterations vary across joints and angular velocities,indicating joint-spe-cific and velocity-dependent characteristics.Individualized rehabilitation programs tailored to joint-specific muscle strength profiles are essential to promote optimal recovery of muscle strength and functional performance after ACLR.
