目的 探讨单纯性先天性中耳畸形的听力学特征.方法 回顾性分析78例(93耳)经鼓室探查确诊为单纯性先天性中耳畸形患者基线资料、术前颞骨高分辨CT(HRCT)、听力学检查与术中所见相关性.结果 HRCT总体阳性率为81.7%.根据镫骨底板固定与否分组,固定组骨导出现Carhart切迹比例显著高于活动组,差异有统计学意义(P<0.05);两组均有明显低频气骨导差(ABG),活动组高频ABG显著大于固定组,差异有统计学意义(P<0.05).结论 单纯性先天性中耳畸形患者纯音听阈表现为中-重度传导性聋,以低频下降为主.聚类分析提示1 000~4 000 Hz ABG、250~1 000 Hz骨导曲线斜率、1 000~4 000 Hz骨导曲线斜率具有预测镫骨底板固定的潜力.若骨导出现Carhart切迹,气导呈平坦-递增型曲线,HRCT阴性,是镫骨底板固定的潜在风险信号.

OBJECTIVE To characterize the audiological features of isolated congenital middle ear anomalies.METHODS A retrospective analysis was conducted on baseline clinical data,preoperative high-resolution temporal bone computed tomography(HRCT),audiological examinations(including pure-tone audiometry),and intraoperative findings in 78 patients(93 ears)diagnosed with isolated congenital middle ear anomalies via tympanotomy.RESULTS The overall HRCT detection rate was 81.7%.Patients were categorized based on intraoperative confirmation of stapes footplate fixation status.The proportion of cases exhibiting a"Carhart notch"was significantly higher in the fixation group than in the mobile group(P<0.05).Both groups displayed significant low-frequency air-bone gaps(ABG),indicative of conductive hearing loss;The mobile group demonstrated significantly larger ABG in the high-frequency range(1 000-4 000 Hz)compared to the fixation group(P<0.05).CONCLUSION Isolated congenital middle ear anomalies typically present as moderate-to-severe conductive hearing loss,predominantly affecting low frequencies.Cluster analysis identified three audiological parameters with predictive potential for stapes footplate fixation:1.ABG magnitude at 1 000-4 000 Hz,2.slope of the bone conduction threshold curve at 250-1 000 Hz,3.slope of the bone conduction threshold curve at 1 000-4 000 Hz.The combination of a Carhart notch,a flat-to-rising air conduction curve,and negative HRCT findings serves as a red-flag profile for stapes footplate fixation.
