阿尔茨海默病(Alzheimer's disease,AD)是一种神经退行性疾病且随着老龄化的到来发病率逐渐升高,临床上通过药物无法从根本上治疗.通过粪菌移植(fecal microbiota transplantation,FMT)来治疗AD是一种新的方法,将供体的健康菌群移植到AD患者的肠道内,AD患者的症状得到改善.在疗效上,动物模型与初步临床案例均显示FMT能在短期内改善认知功能、减轻Aβ蛋白沉积与tau蛋白磷酸化并缓解神经炎症.供体质量、下消化道给药路径和高频强化给药方案被证实是优化疗效的重要调控因素,联合蛋氨酸限制饮食等辅助手段可进一步增效.然而FMT的AD治疗中仍面临一些挑战,如粪菌属性本身的疗效难控和机制不明确,短期与长期安全性风险,监管与规模化生产壁垒.综述了FMT治疗AD的研究现状、疗效、关键参数并描述了FMT治疗AD面临的挑战和未来展望,以期为FMT进一步研发和应用提供理论基础.

Alzheimer's disease(AD)is a neurodegenerative disease,and with the advent of aging,the incidence rate gradually increases,which can not be fundamentally treated by drugs clinically.Fecal microbiota transplantation(FMT)is a new method for treating Alzheimer's disease,which involves transplanting healthy microbiota from donors into the intestines of AD patients and improving their symptoms.This review summarizes the current research status of FMT treatment for AD:both animal models and preliminary clinical cases have shown that FMT can improve cognitive function,reduce A β protein deposition and tau protein phosphorylation,and alleviate neuroinflammation in the short term in terms of efficacy.The quality of the donor,the lower gastrointestinal administration pathway,and the high-frequency intensified administration regimen have been proven to be important regulatory factors for optimizing therapeutic efficacy,and combined with auxiliary measures such as methionine restriction diet can further enhance efficacy.However,there are still some challenges in the treatment of AD with FMT,such as difficulty in controlling the efficacy and unclear mecha-nisms of fecal microbiota properties,short-term and long-term safety risks,regulatory and large-scale production bar-riers.This article reviews the current research status,efficacy,key parameters,and describes the challenges and future prospects of FMT treatment for AD,in order to provide a theoretical basis for further research and application of FMT.
