声触诊组织量化成像视域下的毫针速刺治疗慢性非特异性腰痛的临床研究OA
Clinical Study of Fast Filiform Needling for CNLBP Based on SWE
目的:基于声触诊组织量化成像技术观察毫针速刺治疗对慢性非特异性腰痛患者肌肉功能的影响,评估其临床疗效.方法:研究对象选自2021 年7 月—2023 年8 月于南京中医药大学连云港附属医院就诊的慢性非特异性腰痛患者,共计104 例.通过区组随机化分配方法,分为试验组与对照组,两组各脱落4 例后每组48 例患者.对照组予以常规针刺治疗,试验组予以毫针速刺法治疗.两组患者均治疗2 周.采用疼痛视觉模拟评分(VAS)评估患者腰部疼痛程度,日本骨科协会腰痛评估量表(JOA)评估患者腰椎功能,肌骨超声常规模式下患侧第3 腰椎处肌肉厚度评估肌肉水肿情况,声触诊组织量化成像模式下剪切波速度(SWV)评估肌肉弹性,并评价临床疗效.结果:试验组总有效率为93.75%(45/48),优于对照组的79.17%(38/48),差异具有统计学意义(P<0.05).治疗后,两组患者疼痛VAS评分降低,JOA评分升高,肌肉厚度降低,SWV降低,差异均具有统计学意义(P<0.01),且试验组各评分改善程度优于对照组,差异具有统计学意义(P<0.05 或0.01).结论:毫针速刺可有效缓解CNLBP患者腰部疼痛,改善患者腰椎功能,减轻局部软组织水肿,恢复肌肉弹性,降低疾病对患者生活能力的影响,相较常规针刺更具优势.
Objective:To evaluate the clinical efficacy of fast filiform needling in treatment of chronic nonspecific low back pain(CNLBP)and to observe its effects on muscle function using shear wave elastography(SWE).Methods:A total of 104 patients with CNLBP were randomly assigned into the study group and the control group,with 48 patients in each group after 4 cases dropped off per group.The control group received conventional acupuncture,whereas the study group received fast filiform needling.Both groups underwent treatment for 2 weeks.The Visual Analogue Scale(VAS)was used to assess lumbar pain severity,and the Japanese Orthopaedic Association(JOA)score was adopted to evaluate lumbar spine function.Muscle thickness at the affected side of the L3 level was measured using conventional musculoskeletal ultrasound to assess muscle edema,while SWE was used to determine shear wave velocity(SWV)to evaluate muscle elasticity.Clinical efficacy was also assessed in the two groups.Results:The total effective rate was 93.75%(45/48)in the study group,which was higher than 79.17%(38/48)in the control group(P<0.05).The VAS scores were decreased,the JOA scores were increased,the muscle thickness were reduced,and the SWV values were decreased in both groups after the treatment(P<0.01);the improvements in the study group were more significant than those in the control group(P<0.05,P<0.01).Conclusion:Fast filiform needling can effectively relieve low back pain in patients with CNLBP,improve lumbar function,reduce local soft tissue edema,restore muscle elasticity,and lessen the impact of the condition on daily life.Compared with conventional acupuncture,it demonstrates superior therapeutic benefits.
