手足十二针联合功能训练对脑卒中后偏瘫患者运动功能及表面肌电图的影响OA
Effects of Hand-Foot Twelve-Needle Combined with Functional Training on Motor Function and sEMG in Patient with Post-Stroke Hemiplegia
目的:探究手足十二针联合功能训练对脑卒中后偏瘫患者运动功能及表面肌电图(sEMG)的影响.方法:选取2022 年1 月—2023 年12 月期间在南京医科大学第四附属医院接受治疗的脑卒中后偏瘫患者100 例作为研究对象,随机数表法分为对照组50 例,予以功能训练;研究组50 例,在对照组基础上予以手足十二针治疗.8 个疗程结束后比较两组疗效、中医症候积分、运动功能及sEMG指标.结果:研究组治疗总有效率(90%,45/50)高于对照组(72%,36/50),差异具有统计学意义(P<0.05).治疗后,两组主症、次症、舌脉症积分以及总分均较同组治疗前降低,且研究组低于对照组,差异具有统计学意义(P<0.05).治疗后,两组上肢Fugl-Meyer运动量表(FMA)、下肢FMA、功能性步态评价表(FGA)以及Berg平衡量表(BBS)评分均较同组治疗前升高,且研究组高于对照组,差异具有统计学意义(P<0.05).治疗后,两组股四头肌、胫骨前肌与腓肠肌的肌电积分值(iEMG)均较同组治疗前升高,且研究组高于对照组,差异具有统计学意义(P<0.05);两组均方根振幅(RMS)均较同组治疗前降低,且研究组低于对照组,差异具有统计学意义(P<0.05).结论:手足十二针联合功能训练治疗效果显著,能改善脑卒中后偏瘫患者运动及平衡能力,促进肌力恢复.
Objective:To explore the effect of hand-foot twelve-needle combined with functional training on motor function and surface electromyography(sEMG)in patients with post-stroke hemiplegia.Methods:A total of 100 patients with post-stroke hemiplegia were randomly divided into the control group(n=50)and the study group(n=50).The control group was treated with functional training;on which basis,the study group was also treated with hand-foot twelve-needle.After eight treatment courses,therapeutic efficacy,TCM symptom scores,motor function,and sEMG parameters were compared between the two groups.Results:The total effective rate was 90%(45/50)in the study group,which was significantly higher than 72%(36/50)in the control group(P<0.05).The scores for primary symptoms,secondary symptoms,tongue and pulse signs,and total TCM symptoms were significantly reduced in both groups after the treatment compared with those before the treatment,with lower scores observed in the study group than in the control group(P<0.05).The scores of upper-limb Fugl-Meyer Assessment(FMA),lower-limb FMA,Functional Gait Assessment(FGA),and Berg Balance Scale(BBS)were significantly increased in both groups after the treatment compared with those before the treatment,and the increases were greater in the study group than in the control group(P<0.05).The integrated electromyography(iEMG)values of the quadriceps femoris,tibialis anterior,and gastrocnemius muscles were significantly increased in both groups after the treatment compared with those before the treatment,with higher values in the study group than in the control group(P<0.05).The root mean square(RMS)amplitudes were significantly decreased in both groups after the treatment compared with those before the treatment,and the values in the study group were lower than those in the control group(P<0.05).Conclusion:Hand-foot twelve-needle combined with functional training therapy shows significant therapeutic efficacy in patients with post-stroke hemiplegia.This combined approach effectively improves motor and balance functions and promotes the recovery of muscle strength.
