Fatty acid metabolic reprogramming as a novel therapeutic target for pulmonary fibrosis:a research progress

肺纤维化(pulmonary fibrosis,PF)是一种慢性进行性间质性疾病,以肺泡结构破坏和ECM过度沉积为主要病理特征,目前尚缺乏有效治疗手段.近年研究表明,脂肪酸代谢(fatty acid metabolism,FAM)重编程在PF发生与发展中起重要作用.该文从脂肪酸摄取、合成与氧化3大核心环节的异常调控机制对PF中FAM重编程的研究进展进行了系统综述.在摄取方面,CD36、FABPs等转运蛋白表达失调导致脂质蓄积与细胞损伤;合成环节中FASN通过Wnt/β-catenin等信号通路调控纤维化进程;氧化代谢中 CPT1A、AMPK、PPARα等关键分子功能紊乱引起脂肪酸氧化障碍,进而导致线粒体功能失调与促纤维化信号激活.在此基础上,该文进一步梳理了针对FAM的干预策略,包括膳食调控、药物靶点(如FAAH、GPR84、PCSK9、miR-33)、天然产物与中药复方以及特定脂肪酸干预等多途径治疗方法,为恢复脂代谢平衡及抗纤维化提供新视角.

Pulmonary fibrosis(PF)is a chronic progressive in-terstitial lung disease,which is mainly characterized by the de-struction of alveolar structure and excessive deposition of extra-cellular matrix.Currently,it lacks effective treatment options.Recent studies have indicated that fatty acid metabolism repro-gramming plays an important role in the pathogenesis and pro-gression of PF.This article systematically reviews the research progress of fatty acid metabolic reprogramming in PF from the abnormal regulatory mechanisms of fatty acid uptake,synthesis and oxidation.In terms of uptake,dysregulated expression of transport proteins such as CD36 and FABPs leads to intracellu-lar lipid accumulation and subsequent cellular injury.Regard-ing synthesis,FASN orchestrates the fibrotic progression via sig-naling pathways including Wnt/β-catenin.In oxidative metabo-lism,functional impairments of key molecules like CPT1A,AMPK,and PPARα result in disrupted fatty acid oxidation,which in turn induces mitochondrial dysfunction and activates profibrotic signaling.On this basis,this review further summa-rizes the intervention strategies targeting fatty acid metabolism,including dietary regulation,drug targets(such as FAAH,GPR84,PCSK9,and miR-33),natural products and Chinese herbal compound prescriptions,and specific fatty acid interven-tions,as well as other multi-modal therapeutic approaches,pro-viding new perspectives for restoring lipid metabolic balance and anti-fibrotic therapy.
