结直肠腺瘤患者消化内镜切除术后再发预测模型构建及预防干预措施分析OA
Construction of a predictive model for recurrence after endoscopic resection of colorectal adenomas and analysis of preventive intervention measures
目的 探讨结直肠腺瘤患者消化内镜切除术后再发预测模型构建及预防干预措施.方法 回顾性收集2021年2月至2022年12月于南京中医药大学附属医院(江苏省中医院)行消化内镜切除术治疗的183例结直肠腺瘤患者的临床资料纳入建模组.收集2023年1月至12月于南京中医药大学附属医院(江苏省中医院)行消化内镜切除术治疗的117例结直肠腺瘤患者的临床资料纳入验证组.随访24个月,记录结直肠腺瘤再发情况.采用多因素Logistic回归分析结直肠腺瘤患者消化内镜切除术后再发的独立危险因素.基于危险因素构建Nomogram预测模型.绘制受试者操作特征曲线(receiver operator characteristic curve,ROC曲线)评估模型的区分度;Hosmer-Lemeshow拟合优度检验评价模型校准度;绘制决策曲线分析(decision curve analysis,DCA)评估模型的临床适用度.结果 建模组183例患者中,62例再发,发生率为33.88%(再发组).单因素分析显示,再发组与未再发组患者性别、BMI、高血压、高脂血症、饮酒史和病变部位比较差异均无统计学意义(均P>0.05),年龄、糖尿病、吸烟史、腺瘤大小、腺瘤数量、病理类型及组织分级组间比较差异均有统计学意义(均P<0.05).多因素Logistic回归分析显示,年龄、吸烟史、腺瘤大小、腺瘤数量、病理类型及组织分级均为结直肠腺瘤患者消化内镜切除术后再发的独立危险因素(均P<0.05).通过ROC曲线对列线图模型进行区分度评估,建模组曲线下面积(area under the curve,AUC)为0.960(95%CI:0.924~0.996),特异度和敏感度分别为95.00%和87.50%;验证组AUC为0.949(95%CI:0.905~0.994),特异度和敏感度分别为95.50%和85.60%.通过Hosmer-Lemeshow拟合优度检验和校准曲线对模型进行校准度评估,结果显示,建模组χ2=5.41,P=0.78>0.05,验证组χ2=7.01,P=0.54>0.05.采用DCA评价模型的临床适用度,建模组和验证组数据集阈值在1%~99%区间内,该模型的净获益曲线同时高于"全部干预"和"全部不干预"两条策略线.结论 结直肠腺瘤患者消化内镜切除术后再发率较高,且受多因素影响,通过构建预测模型有助于早期识别高危患者,采取针对性预防干预措施.
Objective To explore the construction of a recurrence prediction model and preventive intervention measures for colorectal adenoma after endoscopic resection.Method Retrospective collection of clinical data from 183 patients with colorectal adenomas who underwent endoscopic resection at The Affiliated Hospital of Nanjing University of Chinese Medicine(Jiangsu Provincial Hospital of Traditional Chinese Medicine)from February 2021 to December 2022 was included in the modeling group.Collect clinical data of 117 patients with colorectal adenomas who underwent endoscopic resection at The Affiliated Hospital of Nanjing University of Chinese Medicine(Jiangsu Provincial Hospital of Traditional Chinese Medicine)from January to December 2023 and include them in the validation group.Follow up for 24 months and record the recurrence of colorectal adenomas.Using multiple Logistic regression analysis to identify independent risk factors for recurrence in patients with colorectal adenoma after endoscopic resection.Build a Nomogram prediction model based on risk factors.Draw receiver operator characteristic curve(ROC curve)to evaluate the discriminative power of the model;Hosmer-Lemeshow goodness of fit test evaluation model calibration degree;draw decision curve analysis(DCA)to evaluate the clinical applicability of the model.Result Among the 183 patients in the modeling group,62 cases had recurrence,with an incidence rate of 33.88%(recurrent group).Univariate analysis showed that there were no significant differences between the recurrent group and the non recurrent group in gender,hypertension,hyperlipidemia,drinking history and lesion location(all P>0.05),but there were significant differences between the age,diabetes mellitus,smoking history,adenoma size,number of adenomas,pathological type and histological grading groups(all P<0.05).Multivariate Logistic regression analysis showed that age,smoking history,adenoma size,number of adenomas,pathological type and histological grading were independent risk factors for recurrence of colorectal adenoma after endoscopic resection(all P<0.05).The ROC curve was used to evaluate the discrimination of the column chart model.The area under the curve(AUC)of the modeling group was 0.960(95%CI:0.924-0.996),with specificity and sensitivity of 95.00%and 87.50%,respectively;the AUC of the validation group was 0.949(95%CI:0.905-0.994),with specificity and sensitivity of 95.50%and 85.60%,respectively.The Hosmer-Lemeshow goodness of fit test and calibration curve were used to evaluate the calibration of the model.The results showed that the consistency index of the modeling group χ2=5.41,P=0.78>0.05,and the validation group χ2=7.01,P=0.54>0.05.The clinical utility of the DCA evaluation model was evaluated,and the threshold values of the modeling and validation datasets were within the range of 1%to 99%.The net benefit curve of the model was higher than both the"all intervention"and"all non intervention"strategy lines.Conclusion The recurrence rate of colorectal adenoma after endoscopic resection is relatively high and is influenced by multiple factors.Constructing a predictive model can help identify high-risk patients early and take targeted preventive interventions.
