目的 探讨肺结节中医证候分布,为临床中医规范化辨治肺结节提供理论依据.方法 收集南京中医药大学常州附属医院呼吸科420例肺结节患者的一般资料及中医四诊信息,采用SPSS27.0软件进行因子分析及聚类分析,运用Lantern5.0软件基于LTM-EAS算法进行隐结构模型分析,综合多元统计分析结果并结合临床总结推断肺结节常见证候规律特点.结果 共收集中医症状条目175个,其中,频次>20的主要症状条目共41个,产生17个公因子.肺结节病位主要在肺、脾、肝,常见病理因素包括虚、痰、湿、气、火、郁、瘀,结合中医知识聚类分析共推断出8种肺结节潜在证型.对频次≥3的96个症状条目进行隐结构模型分析,产生17个隐变量,综合聚类分析得到9种肺结节潜在证型,结合临床总结推断出7种肺结节常见证型及相应症状.结论 肺结节中医证候虚证常见肺气虚证、肺脾气虚证、气阴两虚证,实证常见痰浊阻肺证、痰热蕴肺证、痰瘀内结证、肝郁气滞证.本研究采用多元统计分析方法从不同维度探讨肺结节常见中医证候规律,为肺结节中医证候规范化研究提供基础.

Objective To explore the distribution of TCM syndromes of pulmonary nodules;To provide theoretical basis for standardized clinical diagnosis and treatment of pulmonary nodules.Methods This study collected general data and TCM for diagnostic information of 420 pulmonary nodules patients from Changzhou Affiliated Hospital of Nanjing University of Chinese Medicine.Factor analysis and clustering analysis were conducted using SPSS 27.0 software,while latent structure modeling was performed with the LTM-EAS algorithm in Lantern 5.0 software.Clinical summaries were combined with multivariate statistical analysis to identify common syndrome patterns of pulmonary nodules.Results A total of 175 TCM symptom items were collected.Among them,41 main symptom items with frequency>20 produced 17 common factors.The pathological locations of pulmonary nodules were primarily associated with lung,spleen and liver organs.Common pathogenic factors included deficiency,phlegm,dampness,qi,fire,stagnation and blood stasis.Combined with TCM knowledge,clustering analysis identified 8 potential TCM syndromes.Latent structure model analysis was performed on 96 symptom items with frequency≥3,and 17 latent variables were generated,yielding 9 potential syndromes through comprehensive clustering.Combined with clinical summary,7 common syndrome types and corresponding symptoms of pulmonary nodules were inferred.Conclusion Lung qi deficiency syndrome,lung spleen qi deficiency syndrome and qi yin deficiency syndrome are common in TCM deficiency syndromes of pulmonary nodules,while phlegm turbid obstructing lung syndrome,phlegm heat accumulating lung syndrome,phlegm and blood stasis internal stagnation syndrome and liver stagnation and qi stagnation syndrome are common in excess syndrome.This study uses multivariate statistical analysis method to explore the common TCM syndrome patterns of pulmonary nodules from different dimensions,and provides the basis for the standardization of TCM syndrome patterns of pulmonary nodules.
