基于"脾主运化"理论探讨"脾-肠-眼"轴在湿性年龄相关性黄斑变性防治中的作用OA

Wet age-related macular degeneration(wAMD)is one of the main causes of irreversible visual impairment in the elderly population.The pathological site is located in the retina.The factors such as aging,smoking,and environment all affect the occurrence of wAMD.The etiology and pathogenesis in Traditional Chinese Medicine(TCM)are mostly attributed to spleen failing to govern transportation and transformation.Dysfunction of spleen governing transportation and transformation leads to dampness and turbidity rising upward,stagnating in the eye,and obstructing the spirit light.Therefore,whether the spleen's function of governing transportation and transformation is normal greatly affects retinal function.Spleen and intestine are functionally interconnected and pathologically related.Gut microbiota,as an important material basis of spleen governing transportation and transformation,can interact with wAMD through metabolic products,inflammation,immune responses,and other pathways.Thus,this paper constructs the spleen-intestine-eye axis,aiming to integrate microbiome studies with the theory of spleen governing transportation and transformation through this axis,explore the mechanisms and preventive measures of wAMD in modern medicine,and provide a theoretical basis for optimizing integrated TCM and Western Medicine diagnostic and therapeutic strategies for wAMD.
