基于"扶阳学说"论治中心性浆液性脉络膜视网膜病变OA
Treating Central Serous Chorioretinopathy Based on the"Yang-supporting Theory"
中心性浆液性脉络膜视网膜病变(CSC)是常见的眼科疾病,其病因病机复杂,且易反复发作,病情迁延,严重损害视功能,西医以激光、眼内注药等方式为主要治疗手段,但因易产生多种不良反应,而使该疾病在治疗时选择多有局限.近年来,中医在治疗CSC方面取得一定成果,并因安全、有效等特点在治疗上逐渐占据优势.中医学认为,CSC的主要病机为水湿聚集,上犯目络,而水湿聚集多因机体阳虚无力运化水液而致,因此本文基于"扶阳学说",提出CSC的病机之一为"阳虚",并分别从脾阳虚及肾阳虚的角度阐释CSC的病机,治以暖脾阳、温肾阳、利水湿,为临床治疗CSC提供新的方法和思路.
Central serous chorioretinopathy(CSC)is a common ophthalmic disease with complex etiology and pathogenesis.It is prone to recurrent attacks and protracted course,which seriously impairs visual function.Western Medicine mainly adopts laser or intravitreal injection and other methods for treatment,but the treatment options are quite limited due to the easy occurrence of various adverse reactions.In recent years,Traditional Chinese Medicine(TCM)has achieved certain results in the treatment of CSC,and has gradually occupied an advantageous position in clinical treatment due to its characteristics of safety and effectiveness.TCM holds that the main pathogenesis of CSC is the accumulation of water dampness that attacks the ocular collaterals upward,and the accumulation of water dampness is mostly caused by the body's Yang deficiency which leads to inability to transport and transform body fluids.Therefore,based on the"Yang-supporting theory",this paper proposes that one of the pathogenesis of CSC is"Yang deficiency",and expounds the pathogenesis of CSC from the perspectives of spleen-Yang deficiency and kidney-Yang deficiency respectively.The treatment is to warm spleen-Yang,tonify kidney-Yang and eliminate water dampness,so as to provide new methods and ideas for the clinical treatment of CSC.
