基于数据挖掘探讨《审视瑶函》中医眼科外治法用药规律OA
Analysis of Medication Rules for External Therapies in Ophthalmology of Shen Shi Yao Han Based on Data Mining
目的 基于数据挖掘技术探析《审视瑶函》记载的中医外治法涉及中药的用药规律，为临床提供中药外治的诊疗思路及用药经验。方法 纳入《审视瑶函》中明确记载的中医外治法涉及的中药，录入Excel 2019软件建立数据库，规范处理中药名称后，统计给药方式。再将数据导入中医传承计算平台V3.5，统计药物四气、五味、归经、功效及使用频次;选用"方剂分析"模块进行聚类分析和关联规则功能分析。结果(1)中药属性:共涉及处方84个，中药206味，累计频次为607次。中药四气以寒性最多(280次，46.13％)，五味以辛味最多(269次，32.18％)，归经以肝经用药最多(346次，21.21％)。(2)给药方式:共涉及11种给药方式，排在前5位的依次为点眼(40次，47.06％)，洗眼(11次，12.94％)，㗜鼻(7次，8.24％)，敷眼(6次，7.06％)，吹鼻、涂敷眼部、搽涂眼部(各5次，5.88％)。(3)中药使用频次:使用频次≥10次的药物(高频中药)共29味，排在前5位的分别为黄连(27次，32.14％)，当归(22次，26.19％)，冰片(19次，22.62％)，防风和朱砂(各17次，20.24％)。(4)中药功效分类:以清热药最多(146次，24.05％)，其次是解表药(98次，16.14％)、活血化瘀药(57次，9.39％)、补虚药(49次，8.07％)、拔毒化腐生肌药(39次，6.43％)。(5)聚类分析可得到4类组方。第Ⅰ类功效为祛风散寒、行气活血，第Ⅱ类功效为清热利湿、去腐生肌，第Ⅲ类功效为清热解毒、消肿止痛，第Ⅳ类功效为清热解毒、明目退翳。(6)中药关联规则分析:共获得药物组合共15组，相关性排在前5位的依次是黄连、荆芥-防风，没药-乳香，细辛-防风，赤芍-当归，珍珠-朱砂。结论《审视瑶函》眼科外治法以清热祛风、解毒活血为纲，用药多寒辛入肝，剂型以点眼、洗眼为主，擅用矿物动物之品，体现了驱邪扶正、异病同治的学术思想。
OBJECTIVE To analyze the medication rules of Chinese medicines involved in the external treatments recorded in Shen Shi Yao Han using data mining technology,and to provide diagnostic ideas and medication experience for clinical external treatment with Chinese medicines.METHODS Chinese medicines explicitly recorded for external treatments in Shen Shi Yao Han were collected and entered into Excel 2019 software to establish a database.After standardizing the Chinese medicine names,administration routes were statistically analyzed.The data were then imported into the Traditional Chinese Medicine Inheritance Computing Platform V3.5 to analyze medicine properties including four natures,five flavors,meridian tropism,efficacy,and usage frequency.The"Prescription Analysis"module was used for cluster analysis and association rule analysis.RESULTS(1)Medicine properties:A total of 84 prescriptions involving 206 Chinese medicines with a cumulative frequency of 607 uses were included.Regarding the four natures,cold-nature Chinese medicines were the most common(280 times,46.13％).Regarding the five flavors,pungent flavor Chinese medicines were the most common(269 times,32.18％).Regarding meridian tropism,Chinese medicines targeting Liver meridian were the most common(346 times,21.21％).(2)Administration routes:Eleven administration routes were involved.The top five were eye dropping(40 times,47.06％),eye washing(11 times,12.94％),nasal insufflation(seven times,8.24％),eye application(6 times,7.06％),and nose blowing,eye coating,eye rubbing(five times each,5.88％).(3)Usage frequency:Twenty-nine medicines were high-frequency Chinese medicines(used≥10 times).The top five were Coptidis rhizoma(27 times,32.14％),Angelicae sinensis radix(22 times,26.19％),Borneolum syntheticum(19 times,22.62％),Saposhnikoviae radix and cinnabaris(17 times each,20.24％).(4)Efficacy classification:Heat-clearing Chinese medicines were the most common(146 times,24.05％),followed by exterior-relieving Chinese medicines(98 times,16.14％),blood-activating and stasis-resolving Chinese medicines(57 times,9.39％),deficiency-tonifying Chinese medicines(49 times,8.07％),and toxin-removing,putrefaction-resolving,and flesh-engendering Chinese medicines(39 times,6.43％).(5)Cluster analysis:Four prescription clusters were identified.Cluster Ⅰ efficacy was dispelling wind and dispersing cold,moving Qi and activating blood.Cluster Ⅱ efficacy was clearing heat and disinhibiting dampness,removing putrefaction and engendering flesh.Cluster Ⅲ efficacy was clearing heat and resolving toxicity,dispersing swelling and stopping pain.Cluster Ⅳ efficacy was clearing heat and resolving toxicity,brightening the eyes and removing screen.(6)Association rule analysis:Fifteen Chinese medicine pairs were obtained.The top five associated pairs were Coptidis rhizoma and Schizonepetae herba-Saposhnikoviae radix;Myrrha-Olibanum;Asari herba-Saposhnikoviae radix;Paeoniae rubra radix-Angelicae sinensis radix;Margarita-Cinnabaris.CONCLUSIONS The external treatments in ophthalmology of Shen Shi Yao Han are guided by the principles of clearing heat,dispelling wind,resolving toxicity,and activating blood.The medicines are mostly cold and pungent,entering liver meridian.The primary dosage forms are eye dropping and eye washing,with adept use of mineral and animal Chinese medicines,reflecting the academic thought of expelling pathogenic factors and supporting healthy Qi,and treating different diseases with the same method.
