YKL-40和CLEC4G在特应性皮炎变化及其与临床特征的相关性中的表达OA
Changes in YKL-40 and CLEC4G Levels in Atopic Dermatitis and Their Correlation with Clinical Features
目的 探讨几丁质酶 3 样蛋白 1(YKL-40)和 C 型凝集素结构域家族 4 成员 G(CLEC4G)在特应性皮炎(AD)中的表达水平,分析其与病情严重程度和临床特征的相关性,并观察度普利尤单抗治疗对二者表达的影响.方法 选取 2022 年 9 月—2024 年 12 月于本院就诊的 115 例 AD 患者为观察组,并根据特应性皮炎评分(SCORAD)进一步分为轻度组(35 例)、中度组(40 例)和重度组(40 例);另选取同期 35 例健康人群作为对照组.检测所有受试者血清 YKL-40、CLEC4G、白细胞介素-4(IL-4)和 IL-13 水平,对中重度 AD 患者给予度普利尤单抗注射治疗,分别在治疗前和治疗4周后检测上述指标的变化.结果 观察组血清 YKL-40 水平显著高于对照组,CLEC4G 水平显著低于对照组,差异有统计学意义(P<0.05);3个亚组患者的血清 YKL-40 和 CLEC4G水平比较,差异均有统计学意义(均P<0.05);血清 YKL-40 水平与 SCORAD 呈正相关(r=0.743,P<0.001),血清 CLEC4G 水平与 SCORAD 呈负相关(r=-0.787,P<0.001).血清 YKL-40 和CLEC4G水平在不同疾病严重程度、AD 家族史和过敏史患者间差异有统计学意义(P<0.05),在其他临床特征中差异无统计学意义(P＞0.05).血清YKL-40 和 CLEC4G水平诊断 AD 的受试者工作特征(ROC)曲线下面积(AUCs)分别为 0.782(95%CI:0.710～0.853)和 0.809(95%CI:0.748～0.871).中重度 AD 患者经度普利尤单抗治疗 4周后,血清 YKL-40、IL-4 和 IL-13 水平显著下降,差异有统计学意义(P<0.05),CLEC4G 水平显著升高,差异有统计学意义(P<0.05).结论 血清YKL-40 和 CLEC4G水平可作为 AD 的潜在标志物,与 AD 的病情严重程度显著相关,且度普利尤单抗治疗可显著调节二者的表达,可能是治疗 AD 的潜在靶点.
Objective In order to investigate the expression of chitinase-3-like protein 1(YKL-40)and C-type lectin domain family 4 member G(CLEC4G)in atopic dermatitis(AD),to evaluate their correlations with disease severity and clinical characteristics,and to observe their changes after dupilumab treatment.Methods A total of 115 patients with AD who attended our hospital from September 2022 to December 2024 were enrolled as the observation group and further subdivided into mild(n=35),moderate(n=40)and severe(n=40)groups according to the scoring atopic dermatitis(SCORAD)index.Thirty-five healthy subjects were selected as the control group.Serum levels of YKL-40,CLEC4G,interleukin-4(IL-4)and IL-13 were measured.Moderate-to-severe AD patients received subcutaneous dupilumab treatment,and the above indicators were detected before treatment and after four weeks of treatment.Results Serum YKL-40 level in the observation group was significantly higher than that in the control group,while CLEC4G level was significantly lower,the difference was statistically significant(both P<0.05).Across the three severity subgroups,YKL-40 and CLEC4G levels had significant statistical differences(all P<0.05).There was a positive correlation between YKL-40 level and SCORAD(r=0.743,P<0.001),and a negative correlation between CLEC4G level and SCORAD(r=-0.787,P<0.001).YKL-40 and CLEC4G level had significant statistical differences in patients with different disease severity,family history of AD,and allergy history(all P<0.05),while no significant statistical differences were observed in other clinical characteristics(all P＞0.05).The areas under the receiver operating characteristic(ROC)curve(AUCs)for AD diagnosis were 0.782(95%CI:0.710-0.853)for YKL-40 and 0.809(95%CI:0.748-0.871)for CLEC4G.After four weeks treatment with of dupilumab,moderate-to-severe AD patients showed significant decreasing in serum levels of YKL-40,IL-4 and IL-13,the differences were statistically significant(P<0.05),accompanied by a significant increasing in CLEC4G level,the difference was statistically significant(all P<0.05).Conclusion Serum level of YKL-40 and CLEC4G can serve as potential biomarkers of AD,which are significantly correlated with the severity of AD.Dupilumab treatment can significantly regulate the expression of YKL-40 and CLEC4G,which may be potential targets for the treatment of AD.
