目的:观察夹脊电针结合跑台训练对坐骨神经离断损伤大鼠胫骨微结构及胫骨神经生长因子(NGF)蛋白表达,T12~L1节段脊髓、损伤侧坐骨神经和脊髓背根神经节(DRG)中NGF和神经生长因子受体A型(TrkA)表达的影响.方法:将180只雄性SD大鼠随机分为正常对照组、模型对照组、跑台组、夹脊电针组、结合组,按干预2、4、8周3个时间点分为3个亚组,每个亚组12只.除正常对照组外,其余各组均采用坐骨神经离断再缝合模型.造模后第3天开始治疗,跑台组行跑台治疗;夹脊电针组于T12~L1节段行电针疏密波治疗;结合组先夹脊电针后跑台训练,治疗时间均每日1次,每周6次.Micro-CT观察胫骨微结构;Western blotting检测胫骨NGF蛋白表达;免疫组化法检测坐骨神经、T12~L1 DRG和脊髓中NGF和TrkA的蛋白表达情况.结果:干预2、4、8周后,跑台组、夹脊电针组、结合组BV/TV、Tb.N、Tb.Th均高于模型对照组,Tb.SP低于模型对照组(P<0.05);结合组优于单独的跑台组和夹脊电针组.干预2、4周后,夹脊电针组、结合组胫骨骨组织中NGF蛋白表达均高于模型对照组(P<0.05),结合组NGF表达显著高于跑台组(P<0.05);干预8周后,结合组胫骨骨组织中NGF表达均高于模型对照组、跑台组和夹脊电针组(P<0.05).跑台组、夹脊电针组、结合组干预4、8周后脊髓NGF、TrkA表达,干预2、4、8周后坐骨神经NGF、TrkA表达,干预2、4、8周后DRG中NGF表达及干预2、4周后DRG中TrkA表达高于模型对照组(P<0.05);干预4、8周后,结合组脊髓、坐骨神经、DRG中NGF、TrkA表达均高于跑台组、夹脊电针组(P<0.05).结论:夹脊电针结合跑台训练可改善大鼠坐骨神经离断后胫骨微结构,可能与胫骨中NGF表达及脊髓、坐骨神经、DRG中NGF、TrkA表达增加有关.

Objective:To investigate the effects of Jiaji electroacupuncture(EA)combined with treadmill training on tibial microstructure and the protein expression of nerve growth factor(NGF)and its receptor,tropomyosin receptor kinase A(TrkA),in the tibia,spinal cord(T12-L1),injured sciatic nerve,and dorsal root ganglion(DRG)of rats with sciatic nerve transection injury.Methods:A total of 180 male SD rats were randomly assigned into five groups:normal control,model control,treadmill,Jiaji EA,and combination groups.Each group was further divided into three subgroups based on the intervention duration(2,4,and 8 weeks),with 12 rats per subgroup.Except for the normal control group,all rats underwent sciatic nerve transection followed by microsurgical re-suturing to establish the injury model.Interventions commenced on the 3rd postoperative day.The treadmill group received treadmill exercise,the Jiaji EA group received EA at the T12-L1 Jiaji points(dense-disperse wave),and the combination group received EA followed by treadmill training.All treatments were performed once daily,six times per week.Tibial microstructure was evaluated using Micro-CT.Tibial NGF protein expression was detected via Western blotting.The protein expressions of NGF and TrkA in the sciatic nerve,T12-L1 spinal cord,and DRG were assessed by immunohistochemistry.Results:After 2,4,and 8 weeks of intervention,the Treadmill,Jiaji EA,and Combination groups exhibited significantly higher BV/TV,Tb.N,and Tb.Th,alongside lower Tb.Sp compared to the model control group;the combination group showed superior improvements compared to the single intervention groups.At weeks 2 and 4,tibial NGF expression was significantly higher in the Jiaji EA and combination groups than in the model control group(P<0.05),with the combined group significantly outperforming the treadmill group(P<0.05).By week 8,the combination group showed the highest tibial NGF expression among all groups(P<0.05).Furthermore,after 4 and 8 weeks,NGF and TrkA expression in the spinal cord,as well as NGF and TrkA in the sciatic nerve and DRG(at various time points),were significantly upregulated in the intervention groups compared to the model control group(P<0.05).Notably,at 4 and 8 weeks,the Combination group demonstrated significantly higher levels of NGF and TrkA across the spinal cord,sciatic nerve,and DRG than the single-treatment groups(P<0.05).Conclusion:Jiaji electroacupuncture combined with treadmill training effectively improves tibial microstructure following sciatic nerve transection in rats.This protective effect may be associated with the upregulated expression of NGF in the tibia and the enhanced NGF/TrkA signaling pathway in the spinal cord,sciatic nerve,and DRG.
