目的:调查分析家庭医生团队的激励偏好,为优化服务供给激励机制、提升基层医疗服务水平提供科学依据.方法:于2025 年7~8 月,对江苏省南京市4 个区的 10 个社区卫生服务中心的家庭医生团队成员开展问卷调查.采用平衡不完全区组设计(BIBD)法设计问卷,收集调查对象对 11 种激励因素的选择偏好.采用条件 Logit 模型分析家庭医生团队成员对激励因素的总体偏好情况,采用 K均值聚类对家庭医生团队成员激励偏好进行异质性分析.结果:家庭医生签约服务供给的主要激励因素是居民需求度(OR=8.22)、成员薪资待遇(OR=6.95)、居民信任度(OR=6.72)、居民依从性(OR=6.26).偏好异质性分析显示,两类家庭医生团队成员在居民需求度、居民依从性、居民信任度、成员薪资待遇、成员绩效考核、政府资源投入上表现出显著的偏好异质性,其中对于居民需求度的偏好差异最大.结论:优化家庭医生签约服务供给激励机制应聚焦居民需求度、居民信任度以及成员薪资待遇等,并通过设计多样化的组合激励措施,激发家庭医生的服务供给动力.

Objective To investigate and analyze the incentive preferences of family doctor teams,and provide a scientific ba-sis for optimizing the incentive mechanism for service provision and enhancing the level of primary-level medical services.Meth-ods From July to August 2025,a questionnaire survey was conducted among members of family doctor teams from 10 community health service centers in four districts of Nanjing,Jiangsu Province.The Balanced Incomplete Block Design(BIBD)method was employed to design the questionnaire,which was used to collect the respondents'preference choices regarding 11 incentive factors.A Conditional Logit model was used to analyze the overall preference of family doctor team members for incentive factors,while K-means clustering was applied to conduct heterogeneity analysis of their incentive preferences.Results The primary incentive fac-tors for the provision of contracted family doctor services were residents'demand level(OR=8.22),members'salary and benefits(OR=6.95),residents'trust level(OR=6.72),and residents'compliance(OR=6.26).The heterogeneity analysis of prefer-ences revealed that two types of family doctor team members exhibited significant preference heterogeneity in terms of residents'de-mand level,residents'compliance,residents'trust level,members'salary and benefits,members'performance assessment,and government resource investment,with the greatest difference in preference for residents'demand level.Conclusion To optimize the incentive mechanism for contracted family doctor service provision,attentions should be focused on factors such as residents'de-mand level,residents'trust level,and members'salary and benefits.Additionally,designing diverse and combined incentive meas-ures can stimulate the motivation of family doctors in service provision.
