目的 探讨Bentall手术患者体外循环期间血液吸附的安全性和有效性.方法 回顾性研究2020年1月至2022年12月期间在体外循环下接受Bentall手术的患者.根据纳入和排除标准,术中使用血液吸附(HA380)装置的患者为HA组(n=225),未使用HA380的患者为对照组(n=179).比较两组患者炎症因子、ICU住院时间、机械通气时间、术后住院时间等.采用SPSS 25.0统计软件进行分析、Graphpad Prism 6.0软件进行制图.结果 HA组患者总住院时间、血管活性药物使用时间均显著短于对照组,差异有统计学意义(P<0.05).HA组体外膜氧合使用率也低于对照组,差异有统计学意义(P<0.05).术后24 h和48 h,HA组白介素(IL)-6、IL-8水平均低于对照组.术后3 d,HA组血小板计数明显低于对照组,差异有统计学意义(P<0.05).HA组术后血浆输注量、术后引流量均高于对照组,差异均有统计学意义(P<0.05).结论 Bentall患者术中使用HA380血液吸附可减少血管活性药物的使用,显著降低术后早期血浆IL-6、IL-8水平,改善预后,显著缩短住院时间.但应避免HA380引起的术后引流量增加.

Objective To explore the safety and effectiveness of hemoadsorption during Cardiopulmonary bypass(CPB)in patients underwent Bentall operation.Methods This is a case-control study,focusing on patients who underwent Bentall op-eration with CPB between January 2020 and December 2022.Patients who used intraoperative hemoadsorption with the HA380 cartridge were in the HA group,and patients who were not used in the control group.According to the inclusion and exclusion criteria,225 patients were in the HA group and 179 in the control group.Inflammatory factors,ICU hospitalization time,me-chanical ventilation time,postoperative hospitalization time,etc.were compared between the two groups.SPSS 25.0 statistical software was used for analysis,and Graphpad prism 6.0 software was used for graphing.Results The duration of hospital stay and vasoactive drug use in the HA group were significantly shorter than those in the control group,and the differences were statistically significant(P<0.01).The ECMO usage in the HA group was less than that in the control group,and the difference was statistically significant(P<0.01).At 24 hours and 48 hours after surgery,the levels of IL-6 and IL-8 in the HA group were lower than those in the control group.And three days after surgery,the platelet values of the HA group were significantly lower than those of the control group,and the difference was statistically significant(P<0.01).The ICU length of stay,postoperative plasma transfusion volume and postoperative drainage volume in the HA group were higher than those in the control group,and the differences were statistically significant(P<0.01).Conclusion In this study,we found that intraoperative use of HA380 can significantly reduce the length of hospital stay in Bentall patients,reduce the use of vasoactive drugs,and improve prognosis,and it can significantly reduce plasma IL-6 and IL-8 levels in the early postoperative period.But we should provide prevention and guidance for avoiding complications caused by HA380 resulting in excessive postoperative drainage.
