膳食饱和脂肪酸与不饱和脂肪酸与女性子宫内膜癌发病风险的关联OA
Association Between Dietary Saturated and Unsaturated Fatty Acids and The Risk of Endometrial Cancer in Women
目的:探讨膳食饱和脂肪酸与不饱和脂肪酸与女性子宫内膜癌发病风险的关系.方法:采用病例对照研究,选取2020年1月—2025年1月治疗的新发子宫内膜癌患者382例作为观察组,同时选取健康志愿者382例作为对照组,比较两组膳食脂肪酸摄入情况、临床基线资料差异,分析子宫内膜癌发生的影响因素.结果:观察组单不饱和脂肪酸(MUFA)摄入比例为(38.89±8.22)%,低于对照组的(48.10±9.03)%(P<0.05),而多不饱和脂肪酸(PUFA)摄入比例、n-6/n-3 比值分别为(32.48±9.05)%和(7.06±1.33),高于对照组的(23.35±8.17)%和(5.69±1.12)(P<0.05);观察组和对照组饱和脂肪酸(SFA)摄入比例差异比较无统计学意义(P>0.05);观察组体质量指数≥24 kg/m2、吸烟、糖尿病、子宫内膜癌家族史、未孕比例分别为 41.62%、31.68%、34.55%、29.58%、28.27%,高于对照组的 22.25%、19.11%、21.20%、5.76%和9.42%(P<0.05).Logistic回归分析结果显示,MUFA摄入比例、PUFA摄入比例、n-6/n-3比值、体质量指数、子宫内膜癌家族史、孕次是子宫内膜癌发生的影响因素(P<0.05),其预测子宫内膜癌发生的ROC曲线下面积为0.910(95%CI:0.862~0.958)(P<0.05),灵敏性和特异性分别为92.90%和76.60%.结论:膳食MUFA、PUFA以及n-6/n-3比值与女性子宫内膜癌发病相关,科学合理的脂肪酸摄入或有助于降低子宫内膜癌发病风险.
[Objective]To explore the relationship between dietary saturated and unsaturated fatty acids and the risk of endometrial cancer in women.[Method]A case-control study was used,382 newly diagnosed endometrial cancer patients treated from January 2020 to January 2025 were selected as the observation group,while 382 healthy volunteers were selected as the control group.The differences in dietary fatty acid intake and clinical baseline data between the two groups were compared to analyze the influencing factors of endometrial cancer occurrence.[Result]The proportion of monounsaturated fatty acid(MUFA)intake in the observation group was(38.89±8.22)%,which was lower than that in the control group(48.10±9.03)%(P<0.05),while the proportion of polyunsaturated fatty acid(PUFA)intake and n-6/n-3 ratio were(32.48±9.05)%and(7.06±1.33),respectively,which were higher than those in the control group(23.35±8.17)%and(5.69±1.12)(P<0.05).There was no statistically significant difference in the intake ratio of saturated fatty acids(SFA)between the observation group and the control group(P>0.05).The body mass index(BMI)≥24 kg/m2,smoking,diabetes,family history of endometrial cancer,and the proportion of non pregnancy in the observation group were 41.62%,31.68%,34.55%,29.58%,and 28.27%,respectively,which were higher than those in the control group 22.25%,19.11%,21.20%,5.76%,and 9.42%(P<0.05).Logistic regression analysis showed that MUFA intake ratio,PUFA intake ratio,n-6/n-3 ratio,body mass index,family history of endometrial cancer,and parity were the influencing factors for the occurrence of endometrial cancer(P<0.05),the area under the ROC curve for predicting the occurrence of endometrial cancer was 0.910(95%CI:0.862-0.958)(P<0.05),with sensitivity and specificity of 92.90%and 76.60%,respectively.[Conclusion]Dietary MUFA,PUFA,and n-6/n-3 ratio are influencing factors for the incidence of endometrial cancer in women.Scientific and reasonable intake of fatty acids can help reduce the risk of endometrial cancer.
