氧化锆全瓷与高强纤维树脂贴面在上前牙缺损修复中的美学效果评估OA
Evaluation of the Aesthetic Effect of Zirconia All-porcelain and High-strength Fiber Resin Veneers in the Restoration of Anterior Tooth Defects
目的:对比分析氧化锆全瓷和高强纤维树脂贴面在上前牙缺损修复中的美学效果.方法:连续选取2022年5月-2023年12月承德市口腔医院口腔科收治的行前牙美学修复的88例患者为研究对象,依据修复方式不同分为氧化锆组(40例,采用氧化锆全瓷修复)和纤维树脂组(48例,采用高强纤维树脂贴面修复).比较两组治疗前后牙龈评价指数、咀嚼功能、白色美学指标及牙周组织指标.治疗后采用定期复诊的方式随访6个月,记录并比较两组随访期间牙龈炎症、牙冠裂纹等并发症发生情况,末次随访时评价修复效果.结果:修复后,两组牙龈评价指标、咀嚼功能、白色美学指标、牙周组织指标较修复前均得到有效改善,其中纤维树脂组龈沟出血指数、菌斑指数评分均低于氧化锆组(P＜0.05),龈沟液少于氧化锆组(P＜0.05),白介素-6、肿瘤坏死因子-α水平低于氧化锆组(P＜0.05).修复后,氧化锆组咀嚼效率和咬合力均大于纤维树脂组(P＜0.05),美国公共卫生协会评价标准(USPHS)评分中修复体边缘密合度、着色、颜色评分均高于纤维树脂组(P＜0.05).随访发现,两组并发症发生率、颜色匹配度Ⅰ级比例差异无统计学意义(P＞0.05),边缘密合度优比例(97.50%vs.83.33%)差异有统计学意义(P＜0.05).结论:在前牙缺损修复中,氧化锆全瓷修复在改善患者咀嚼功能、提高牙体美观性方面更有优势;高强纤维树脂贴面修复对牙周组织的损伤更小,更有助于牙龈恢复.
Objective The white aesthetic effects of zirconia porcelain restoration and high strength fiber resin veneer restoration were compared and analyzed.Methods Eighty-eight patients undergoing cosmetic restoration of anterior teeth in Chengde City Stomatological Hospital from May 2022 to December 2023 were selected successively,according to different restoration methods,they were divided into zirconia group(40 cases,zirconium oxide all-ceramic restoration)and fiber resin group(48 cases,high strength fiber resin veneer restoration).Gingival evaluation index,masticatory function,white aesthetic index and periodontal tissue index were compared between the two groups before and after treatment.The patients were followed up regularly for 6 months after treatment,the complications such as gingivitis and crown crack were recorded and compared between the two groups,the restoration effect was evaluated at the last follow-up.Results After repair,the gingival evaluation indexes,masticatory function,white aesthetic indexes and periodontal tissue indexes in the zirconia and fibroresin groups were significantly improved compared with those before repair,the bleeding index and plaque index scores in the fibroresin group were lower than those in the zirconia group(P＜0.05),gingival crevicular fluid was lower than that in the zirconia group(P＜0.05),the levels of interleukin-6 and tumor necrosis factor-α were lower than those in zirconia group(P＜0.05).The masticatory efficiency of zirconia group was higher than that of fiber resin group(P＜0.05),the biting force was higher than that of fiber resin group(P＜0.05),and the edge fit,coloration and color scores of the restoration in USPHS group were higher than those in fiber resin group(P＜0.05).Follow-up showed that there was no significant difference in complication rate and color matching grade I ratio between zirconia group and fiber resin group(P＞0.05).There was significant difference in the proportion of superior edge close fitting(97.50%vs.83.33%)(P＜0.05).Conclusion In cosmetic restoration of anterior teeth,zirconia all-ceramic restoration has more advantages in improving masticatory function and dental aesthetics,high-strength fiber resin veneer restoration has less damage to periodontal tissue and is more conducive to gingival recovery.
