仙方活命饮加减坐浴促进肛周脓肿合并HIV/AIDS患者术后创面愈合的研究OA

Objective To explore the effect of modified Xianfang Huoming Decoction sitz bath on postop-erative wound healing in patients with perianal abscess complicated with HIV/AIDS.Methods A total of 90 patients with perianal abscess complicated with HIV/AIDS who were hospitalized in Affiliated Public Health Clinical Center of Shandong University from September 2023 to August 2025 were enrolled and randomly divided into 3 groups,with 30 cases in each group.All three groups received routine antiviral treatment,and after operation,they were given potassium permanganate,Compound Jingjie Formula for Fumigation and Washing and Xianfang Huoming Decoction sitz bath respectively.The pain,bleeding,edema,exudation and wound healing time of the three groups were compared on the 2nd,7th and 14th days after operation.Results 1)Before sitz bath on the 2nd day after operation,there were no significant differences in the scores of wound pain,bleeding,edema and exudation among the three groups(P>0.05).2)On the 7th day after opera-tion,the scores of all symptoms in the three groups were significantly lower than those on the 2nd day after operation(P<0.05).In the intergroup comparison,there was no significant difference in bleeding score among the three groups(P>0.05),while significant differences were observed in pain,edema and exudation scores(P<0.05 or P<0.001),and the scores of each indicators in the Xianfang Huoming Decoction group were lower than those in the potassium permanganate group and the Compound Jingjie Formula for Fumigation and Wash-ing group(P<0.05).However,only the pain score between the potassium permanganate group and the Com-pound Jingjie Formula for Fumigation and Washing group was significantly different(P<0.05),and there were no significant differences in the scores of other indicators(P>0.05).3)On the 14th day after operation,the scores of pain,bleeding,edema and exudation in the three groups were further lower than those on the 7th day after operation(P<0.05).In the intergroup comparison,there was no significant difference in bleeding score among the three groups(P>0.05),while significant differences were observed in pain,edema and exudation scores(P<0.05 or P<0.001).The pain and edema scores in the Xianfang Huoming Decoction group were lower than those in the potassium permanganate group and the Compound Jingjie Formula for Fumigation and Washing group(P<0.05),and the exudation score was significantly lower than that in the potassium perman-ganate group(P<0.05),but there was no significant difference with the Compound Jingjie Formula for Fumi-gation and Washing group(P>0.05).There were no significant differences in the scores of all indicators between the potassium permanganate group and the Compound Jingjie Formula for Fumigation and Washing group(P>0.05).4)There were significant differences in wound healing time among the three groups(P<0.05),among which the Xianfang Huoming Decoction group had the shortest time and the potassium perman-ganate group had the longest time.Conclusion Modified Xianfang Huoming Decoction sitz bath can effec-tively reduce postoperative pain,edema and exudation in patients with perianal abscess complicated with HIV/AIDS,promote wound healing,and help reduce the risk of occupational exposure of medical staff.
