目的:探讨透脓连理汤对脾虚湿困证溃疡性结肠炎(UC)大鼠模型结肠局部损伤及组织病理学的影响.方法:借鉴环境与饮食干预联合2,4,6-三硝基苯磺酸钠(TNBS)/乙醇灌肠法诱导制作UC大鼠模型.将大鼠分为空白组、模型组、美沙拉秦(SASP)组和透脓连理汤低、中、高剂量组.低、中、高剂量组分别按10.8、21.6、43.2 g/kg予透脓连理汤灌胃,SASP组按0.5 g/kg予SASP灌胃,空白组和模型组按10 mL/kg予0.9%生理盐水灌胃.予上述对应处理15 d后进行形态观察和评分.结果:1)疾病活动指数(DAI)评分:模型组较空白组DAI评分明显升高(P<0.05);各中药组(即透脓连理汤低、中、高剂量组)及SASP组较模型组DAI评分均显著降低(P<0.01),各中药组及SASP组之间差异均无统计学意义(P>0.05).2)结肠组织损伤形态观察和评分:模型组较空白组评分显著升高(P<0.01);各中药组及 SASP组评分较模型组显著降低(P<0.01),各中药组及SASP组之间差异均无统计学意义(P>0.05).3)结肠组织病理损伤程度和评分:模型组较空白组病理学评分显著升高(P<0.01),且可见结肠黏膜结构破坏,杯状细胞减少,隐窝结构破坏、排列紊乱,大量炎性细胞渗出浸润;各中药组及SASP组结肠组织病理形态较模型组显著改善,且评分降低(P<0.05或P<0.01),各中药组及SASP组之间差异均无统计学意义(P>0.05).结论:透脓连理汤具有抗炎及促进组织修复作用,可改善脾虚湿困证UC大鼠结肠组织局部损伤及病理变化.各剂量组未见明显差异,可能与样本量较小有关,有待未来大样本、多中心进一步探讨.

Objective To investigate the effects of Tounong Lianli Decoction on colonic local injury and his-topathology in rat models with ulcerative colitis(UC)of spleen deficiency and dampness retention syndrome.Methods The UC rat model was established by combining environmental and dietary interventions with 2,4,6-trinitrobenzenesulfonic acid(TNBS)/ethanol enema.The rats were randomly divided into the blank group,model group,mesalazine(SASP)group,and low-,medium-,and high-dose Tounong Lianli Decoction groups.Rats in the low-,medium-,and high-dose groups were given Tounong Lianli Decoction by gavage at doses of 10.8,21.6,and 43.2 g/kg,respectively.Those in the SASP group received mesalazine by gavage at a dose of 0.5 g/kg.Rats in the blank group and model group were administered 0.9%normal saline by gavage at a dose of 10 mL/kg.After 15 consecutive days of corresponding interventions,morphological observation and scoring were performed.Results 1)Disease Activity Index(DAI)scores:Compared with the blank group,the DAI score of the model group was significantly increased(P<0.05).Compared with the model group,the DAI scores of all Chinese medicinal groups(low-,medium-,and high-dose Tounong Lianli Decoc-tion groups)and the SASP group were significantly decreased(P<0.01),with no statistically significant dif-ferences among these Chinese medicinal groups and the SASP group(P>0.05).2)Morphological observa-tion and scoring of colonic tissue injury:The score of the model group was significantly higher than that of the blank group(P<0.01).The scores of all Chinese medicinal groups and the SASP group were significantly lower than that of the model group(P<0.01),and there were no statistically significant differences among these Chinese medicinal groups and the SASP group(P>0.05).3)Pathological damage degree and scoring of colonic tissues:Compared with the blank group,the pathological score of the model group was significantly increased(P<0.01),accompanied by destruction of colonic mucosal structure,reduction of goblet cells,dam-age and disordered arrangement of crypt structures,and massive exudation and infiltration of inflammatory cells.The pathological morphology of colonic tissues in all Chinese medicinal groups and the SASP group was significantly improved compared with the model group,with decreased scores(P<0.05 or P<0.01),and no statistically significant differences were observed among these Chinese medicinal groups and the SASP group(P>0.05).Conclusion Tounong Lianli Decoction has an anti-inflammatory effect and can promote tissue repair,which can improve colonic local injury and pathological changes in UC rats of spleen deficiency and dampness retention syndrome.No significant differences were found among the different-dose groups,which may be related to the small sample size,and further exploration with large samples,multicenter studies is war-ranted in the future.
