目的:青光眼是全球首位不可逆致盲眼病.本研究基于 2021 年全球疾病负担(Global Burden of Disease,GBD)数据库,分析 1990-2021 年中国大陆地区青光眼致盲的流行病学特征及趋势,为制定防盲策略提供科学依据.方法:从 GBD 2021 数据库提取 1990-2021 年中国大陆地区青光眼致盲及人口学数据.采用 Joinpoint 回归模型分析青光眼致盲负担的时间趋势,计算年度百分比变化(annual percentage change,APC)、平均年度百分比变化(average annual percentage change,AAPC)及其 95%置信区间(confidence interval,CI).通过年龄-时期-队列模型评估年龄、时期和出生队列对致盲负担的影响,计算净漂移、局部漂移、时期相对风险、队列相对风险及拟合纵向年龄曲线.结果:1990-2021 年间,中国青光眼致盲总病例数有所增加,但致盲率呈显著下降趋势,从 1990 年的 56.83/10 万(不确定性区间:45.93~69.96)降至 2021 年的 23.32/10 万(不 确 定 性 区 间:18.46~28.84),AAPC 为-2.63(95%CI:-2.75~-2.50,P<0.001).男性青光眼致盲负担高于女性,但男性致盲负担下降趋势更为明显.青光眼致盲风险随年龄增长而上升,时期风险和出生队列风险均呈改善趋势.结论:1990 年以来,中国青光眼致盲负担显著下降,反映了防盲工作和技术进步的成效.未来需进一步优化全人群眼健康管理策略,以持续降低青光眼致盲风险.

Objective:Glaucoma is the leading cause of irreversible blindness worldwide.This study aimed to use the Global Burden of Disease(GBD)2021 database to analyze the epidemiological patterns of glaucoma-related blindness in the Chinese mainland from 1990 to 2021,providing evidence for the development of blindness prevention strategies.Methods:Data on glaucoma-related blindness and demographics information from the Chinese mainland were extracted from the GBD 2021 database.Joinpoint regression analysis was used to evaluate the temporal trends in the burden of glaucoma-related blindness.This was achieved by calculating the annual percentage change(APC),the average annual percentage change(AAPC),and their corresponding 95%confidence intervals(CI).Additionally,the age-period-cohort model was employed to assess the impacts of age,period,and cohort on the burden of blindness.This was done by calculating net drift,local drift,period relative risk,cohort relative risk,and fitted longitudinal age curves.Results:Between 1990 and 2021,the total number of cases of glaucoma-related blindness in the Chinese mainland increased.However,the prevalence rate of blindness decreased significantly,dropping from 56.83 cases per 100,000 population(uncertainty interval[UI]:45.93 to 69.96)in 1990 to 23.32 cases per 100,000 population(UI:18.46 to 28.84)in 2021,with an AAPC of-2.63(95%CI:-2.75 to-2.50,P<0.001).The burden of glaucoma-related blindness was higher among males than among females,but the decline in the prevalence of blindness was more marked in males.The risk of glaucoma-related blindness increased substantially with age,while risks associated with the period and cohort showed a decreasing trend.Conclusions:Since 1990,the burden of glaucoma-related blindness in the Chinese mainland has witnessed a significant reduction,which reflects the success of blindness prevention efforts.Continuously optimizing comprehensive strategies for eye health management is crucial to further reduce the incidence of blindness associated with glaucoma.
