目的:本研究旨在探讨电针调控脊髓背角电压门控钙通道α2δ-1 亚基(α2δ-1)、N-甲基-D-天冬氨酸受体2B 亚型(NR2B)及细胞外信号调节激酶 5(ERK5)表达,改善神经病理性疼痛(NP)大鼠的疼痛及伴发抑郁行为机制.方法:选取 40 只雄性 SD 大鼠,随机均分为空白组、模型组、假电针组、电针组,每组 10 只.除空白组以外,其余各组大鼠均建立坐骨神经慢性压迫损伤(CCI)模型.造模第 7 天,电针组于右侧环跳、阳陵泉行电针治疗,假电针组非穴位皮下浅刺且不通电,每次 30 min,连续干预 7 d.观察各组大鼠机械缩足阈值(PWMT)、热缩足潜伏期(PWTL)、蔗糖偏好率、旷场实验运动总距离、穿越方格数.运用 Western blot、RT-qPCR 技术检测脊髓背角α2δ-1、NR2 B、ERK5 的蛋白及 mRNA 表达水平.结果:与空白组相比,模型组大鼠 PWMT 和 PWTL 值、蔗糖偏好率及旷场实验运动总距离、穿越方格数降低(均 P<0.05),脊髓背角中α2δ-1、NR2B 和 ERK5 的蛋白及 mRNA 表达水平升高(均 P<0.05).与模型组相比,电针组大鼠 PWMT 和 PWTL 值、蔗糖偏好率、旷场实验运动总距离、穿越方格数升高(均 P<0.05).脊髓背角α2δ-1、NR2B 和 ERK5 的蛋白及 mRNA 表达水平降低(均 P<0.05).与模型组相比,假电针组上述改变不明显(P>0.05).结论:电针可有效缓解 CCI 模型大鼠痛觉超敏并改善抑郁行为,其作用机制可能与下调脊髓背角α2δ-1 和 NR2B 的异常表达及干预 ERK5 通路有关.

Objective:To explore mechanism by which electroacupuncture regulates expression of voltage-gated calcium channelα2δ-1 subunit(α2δ-1),N-methyl-D-aspartate receptor 2B subtype(NR2B),and extracellular signal-regulated kinase 5(ERK5)in the spinal dorsal horn to alleviate neuropathic pain(NP)and associated depressive be-haviors in rats.Methods:Forty male SD rats were randomly divided into four groups:blank group,model group,the sham electroacupuncture group,and the electroacupuncture group,with 10 rats in each group.Except for the blank group,the other groups were subjected to chronic constriction injury(CCI)of the sciatic nerve.On the 7th day after modeling,the electroacupuncture group received electroacupuncture at the right Huantiao and Yanglingquan points,while the sham electroacupuncture group received superficial subcutaneous needling at non-acupoints without electri-cal stimulation,for 30 minutes each time,for 7 consecutive days.The mechanical paw withdrawal threshold(PWMT),thermal paw withdrawal latency(PWTL),sucrose preference rate,total distance traveled in the open field test,and the number of grid crossings were observed in each group.Western blot and RT-qPCR techniques were used to detect protein and mRNA expression levels ofα2δ-1,NR2B,and ERK5 in the spinal dorsal horn.Results:Compared with the blank group,the PWMT and PWTL values,sucrose preference rate,total distance traveled in the open field test,and the number of grid crossings in the model group decreased(all P<0.05),and the protein and mRNA ex-pression levels of α2δ-1,NR2B,and ERK5 in the spinal dorsal horn increased(all P<0.05).Compared with the model group,the PWMT and PWTL values,sucrose preference rate,total distance traveled in the open field test,and the number of grid crossings in the electroacupuncture group increased,and the protein and mRNA expression levels ofα2δ-1,NR2B,and ERK5 in the spinal dorsal horn decreased(all P<0.05).Compared with the model group,the above changes in the sham electroacupuncture group were not significant(P>0.05).Conclusion:Electroacupuncture can effectively alleviate hyperalgesia and improve depressive behaviors in CCI model rats,and its mechanism may be related to the down-regulation of abnormal expression of α2δ-1 and NR2B in the spinal dorsal horn and the interven-tion of the ERK5 pathway.
