补中益气汤对腰椎间盘退变大鼠作用机制OA
Mechanism of Buzhong Yiqi decoction on lumbar intervertebral disc degeneration in rats
目的:探讨补中益气汤调控 C-C 基序趋化因子配体 2(CCL2)/C-C 基序趋化因子受体 2(CCR2)通路缓解腰椎间盘突出症(LDH)大鼠腰椎间盘退变.方法:使大鼠自体髓核接触神经根硬膜外腔以构建 LDH 模型,并分为模型组、补中益气汤低剂量组、补中益气汤高剂量组、补中益气汤高剂量联合 GW0742 组,每组 12 只,另取 12 只大鼠作为假手术组.热辐射法和机械痛敏法评估大鼠疼痛行为,ELISA 检测血清炎症和疼痛因子水平,HE 染色检测椎间盘病理变化并以 Masuda 评分评估大鼠腰椎间盘退变程度,免疫荧光法检测背根神经节组织中 Iba-1、GFAP 蛋白表达水平,RT-qPCR 和 Western blot 检测背根神经节组织中 CCL2、CCR2 mRNA 和蛋白表达水平.结果:模型组与假手术组相比,大鼠椎间盘髓核皱缩,髓核细胞明显减少,纤维环扭曲破裂,Iba-1 和 GFAP 蛋白表达水平升高,Masuda 评分、TNF-α、IL-1β、NPY 水平、CCL2、CCR2 mRNA 及其蛋白表达量增加,而足底 PWL、PWT 及 5-HT 水平降低(均 P<0.05).补中益气汤低剂量组、补中益气汤高剂量组与模型组相比,大鼠髓核形态和细胞数目恢复,扭曲破裂的纤维环减少,Iba-1 和 GFAP 蛋白表达降低,Masuda 评分、TNF-α、IL-1β、NPY 水平、CCL2、CCR2 mRNA及其蛋白表达量减少,而足底 PWL、PWT 及 5-HT 水平升高,且补中益气汤高剂量组指标变化幅度高于补中益气汤低剂量组(P<0.05).补中益气汤高剂量联合 GW0742 组与补中益气汤高剂量组相比,大鼠腰椎间盘病理损伤加重,且各项指标出现明显好转(均 P<0.05).结论:补中益气汤通过抑制 CCL2/CCR2 通路缓解 LDH 大鼠腰椎间盘退变及疼痛行为.
Objective:To explore effect of Buzhong Yiqi decoction on alleviating intervertebral disc degeneration in rats with lumbar disc herniation(LDH)by regulating the C-C motif chemokine ligand 2(CCL2)/C-C motif che-mokine receptor 2(CCR2)pathway.Methods:LDH models were established by exposing the nucleus pulposus of rats to the epidural space of the nerve root,and the rats were divided into model group,low-dose Buzhong Yiqi decoction group,high-dose Buzhong Yiqi decoction group,and high-dose Buzhong Yiqi decoction combined with GW0742 group,with 12 rats in each group.Another 12 rats were used as the sham operation group.Thermal radiation and me-chanical pain sensitivity methods were used to evaluate the pain behavior of rats.ELISA was used to detect the levels of inflammatory and pain factors in serum.HE staining was used to detect the pathological changes of intervertebral discs and Masuda score was used to evaluate the degree of lumbar intervertebral disc degeneration.Immunofluores-cence was used to detect the expression levels of Iba-1 and GFAP proteins in dorsal root ganglion tissues.RT-qPCR and Western blot were used to detect the expression levels of CCL2 and CCR2 mRNA and proteins in dorsal root ganglion tissues.Results:Compared with the sham operation group,the model group showed atrophy of the nucleus pulposus,significant reduction in the number of nucleus pulposus cells,distortion and rupture of the annulus fibro-sus,increased expression levels of Iba-1 and GFAP proteins,increased Masuda score,TNF-α,IL-1β,NPY levels,CCL2 and CCR2 mRNA and protein expression levels,and decreased paw withdrawal latency(PWL),paw withdraw-al threshold(PWT)and 5-HT levels(all P<0.05).Compared with the model group,the low-dose Buzhong Yiqi de-coction group and the high-dose Buzhong Yiqi decoction group showed recovery of the morphology and cell number of the nucleus pulposus,reduction in distorted and ruptured annulus fibrosus,decreased expression levels of Iba-1 and GFAP proteins,decreased Masuda score,TNF-α,IL-1β,NPY levels,CCL2 and CCR2 mRNA and protein expression levels,and increased PWL,PWT and 5-HT levels.The changes in the high-dose Buzhong Yiqi decoction group were more significant than those in the low-dose Buzhong Yiqi decoction group(P<0.05).Compared with the high-dose Buzhong Yiqi decoction group,the high-dose Buzhong Yiqi decoction combined with GW0742 group showed aggrava-ted pathological damage to lumbar intervertebral discs,but significant improvement in indicators(all P<0.05).Conclusion:Buzhong Yiqi decoction alleviates lumbar intervertebral disc degeneration and pain behavior in LDH rats by inhibiting the CCL2/CCR2 pathway.
