目的 分析放射剂量和影像组学预测乳腺癌放疗患者发生放射性皮炎风险的相关性.方法 选取2021 年3 月至2024 年3 月在我院进行放疗的乳腺癌患者285 例进行分析.所有患者治疗前均进行了放射剂量和影像组学分析,然后给予放射治疗.根据患者放疗期间是否发生放射性皮炎分为皮炎组和对照组.比较两组临床资料.应用多因素 Lo-gistic 回归分析相关因素与乳腺癌放疗患者发生放射性皮炎风险的关系.结果 285 例患者有3 例脱落,剩余282 例患者115 例发生放射性皮炎(皮炎组),放射性皮炎发生率为40.78%.两组患者一般资料、放射总剂量等比较,差异无统计学意义(P>0.05);皮炎组计划靶区体积、靶区平均放射剂量、纹理特征参数(全角度集群突出方差)高于对照组,皮炎组形态学参数(表容比)、直方图参数(均匀度)低于对照组,差异有统计学意义(P<0.05).多因素 Logistic 回归分析显示,计划靶区体积(OR=1.009)、靶区平均放射剂量(OR=1.201)、纹理特征参数(全角度集群突出方差)(OR=1.741)是乳腺癌放疗患者发生放射性皮炎的独立危险因素;形态学参数(表容比)(OR=0.990)、直方图参数(均匀度)(OR=0.014)是乳腺癌放疗患者发生放射性皮炎的保护性因素.列线图预测乳腺癌放疗患者放射性皮炎发生相关影响因素的 C-index 为0.926(95%CI 0.865~0.953),校正曲线显示列线图模型预测放射性皮炎发生风险与内部抽样的相关性较好.结论 计划靶区体积、靶区平均放射剂量、全角度集群突出方差等因素构建的列线图可有效预测乳腺癌放疗患者放射性皮炎的发生风险,具有良好的临床应用价值.

Objective To analyze the correlation between radiation dose parameters and radiomics in predicting the risk of radiation dermatitis in breast cancer patients undergoing radiotherapy.Methods A total of 285 breast cancer patients who re-ceived radiotherapy at our hospital from March 2021 to March 2024 were enrolled.All patients underwent radiation dose calculation and radiomics analysis before treatment.Patients were divided into radiation dermatitis group and control group according to the oc-currence of radiation dermatitis during radiotherapy.Clinical characteristics and imaging parameters were compared between the two groups.Multivariable Logistic regression was performed to identify independent factors associated with radiation dermatitis.Results Among 285 patients,3 cases dropped out,and among the remaining 282 cases,115 cases developed radiation dermatitis(the der-matitis group),with a radiation dermatitis incidence rate of 40.78%.There was no statistically significant difference in the general data and total radiation dose between the two groups of patients(P>0.05).However,the planned target volume,average radiation dose and texture feature parameters in the dermatitis group were higher than those in the control group(P<0.05).Morphological parameters-surface volume ratio and histogram parameter-uniformity in the dermatitis group were lower than those in the control group(P<0.05).Multivariate Logistic regression analysis revealed that the planned target volume(OR=1.009),the average ra-diation dose of the target area(OR=1.201),and the texture feature parameter(the variance of the full-angle cluster)(OR=1.741)were independent risk factors for radiation dermatitis in breast cancer radiotherapy patients;the morphological parameter(the ratio of surface area)(OR=0.990)and the histogram parameter(the uniformity)(OR=0.014)were protective factors for radiation dermatitis in breast cancer radiotherapy patients.The C-index of the nomogram constructed for predicting radiation derma-titis in breast cancer patients undergoing radiotherapy was 0.926(95%CI 0.865~0.953).The calibration curve indicated a good correlation between the risk of radiation dermatitis predicted by the nomogram model and the internal sampling results.Conclusion The nomogram based on planned target volume,mean target dose,and all-angle cluster prominence variance can effectively pre-dict the risk of radiation dermatitis in breast cancer patients undergoing radiotherapy,with favorable clinical application value.
