目的 基于美国国家健康与营养调查(National Health and Nutrition Examination Survey,NHANES)数据分析相对脂肪量(relative fat mass,RFM)与超重和肥胖成年女性压力性尿失禁(stress urinary incontinence,SUI)的关系.方法 使用2011-2018年NHANES数据,对6 190例年龄≥20岁的超重及肥胖女性进行横断面分析.通过研究对象的身高、腰围计算RFM.通过研究对象的自我报告获取其SUI病史.按照研究对象RFM的四分位数将其分为Q1~Q4组.采用加权Logistic回归分析,平滑曲线拟合以及亚组分析来探讨RFM与SUI的关系.结果 本研究共纳入6 190例参与者,其中2 746(47.18%)例有SUI.加权Logistic回归分析显示,在完全调整模型(模型3)中,RFM水平与SUI风险独立相关(OR=1.06,95%CI:1.03~1.08,P=0.000 4).平滑曲线拟合结果显示,RFM与SUI风险呈正线性关系,即随着RFM水平的升高,SUI的患病风险增加.亚组分析结果显示RFM与SUI的关系在抑郁症史上存在交互作用(交互作用P<0.05).结论 在美国超重和肥胖的成年女性中,较高的RFM水平与SUI的患病风险增加相关.RFM具有成为评估SUI风险指标的潜能.肥胖亦是中国女性SUI的危险因素,该发现可为中国同类人群SUI风险预测提供参考,但其适用性需进一步研究验证.

Objective Exploring the association between relative fat mass(RFM)and stress urinary incontinence(SUI)in overweight and obese adult women based on the National Health and Nutrition Examination Survey(NHANES).Methods Using data from the 2011-2018 NHANES,a cross-sectional analysis was conducted on 6 190 overweight or obese women aged ≥ 20 years.The RFM was calculated based on the participants' height and waist circumference.The history of SUI was obtained through self-reports.The participants were divided into four groups according to the quartiles(QI-Q4)of RFM values.Weighted logistic regression analysis,smoothing curve fitting,and subgroup analysis were used to investigate the relationship between RFM and SUI.Results A total of 6 190 participants were included in this study,of whom 2 746(47.18%)had SUI.Weighted logistic regression analysis revealed that,in the fully adjusted model(model 3),RFM was independently associated with the risk of SUI(OR=1.06,95%CI:1.03-1.08,P=0.0004).Smoothing curve fitting indicated that RFM was positively linearly associated with the risk of SUI,meaning that,as RFM increased,the risk of SUI increased too.Subgroup analysis revealed that there was an interaction between RFM and SUI in relation to a history of depression(P for interaction<0.05).Conclusion Among overweight and obese adult women in the United States,higher levels of RFM are associated with an increased risk of SUI.RFM has the potential to serve as a biomarker for assessing the risk of SUI.Given that obesity is also a risk factor for SUI in Chinese women,this finding may inform SUI risk prediction in similar Chinese populations;however,its applicability requires further validation.
