早期体外膈肌起搏对脓毒症患者膈肌功能影响的随机对照研究OA

Objective To explore whether early external diaphragm pacing intervention can effectively inhibit diaphragmatic dysfunction in sepsis and thereby prevent the occurrence of respiratory failure.Methods A prospective,randomized controlled,single-center study design was adopted.Sixty non-respiratory failure sepsis patients admitted to Shanghai Tongji Hospital from January 2022 to April 2023 were selected.All patients received standard sepsis guideline treatment.The intervention group(n=30)received external diaphragm pacing treatment twice a day for 30 minutes each time for one week on this basis.Results After one week of intervention,the decrease in oxygenation index in the control group was significantly greater than that in the intervention group(P＜0.05);the diaphragm activity during quiet breathing and deep breathing,diaphragm thickness at the end of quiet inspiration and deep inspiration,and diaphragm thickening rate in the intervention group were significantly better than those in the control group(P＜0.05).However,there were no statistically significant differences between the two groups in biochemical indicators,the incidence of respiratory failure during hospitalization,8-week all-cause mortality/readmission rate,and 8-week pneumonia re-visit rate(P＞0.05).Conclusion Early external diaphragm pacing can effectively improve the oxygenation index and diaphragm function of sepsis patients,but its impact on reducing the incidence of respiratory failure and long-term prognosis still needs further study.
