散发性肌萎缩侧索硬化症的外周免疫特征OA
Peripheral immune characteristics of sporadic amyotrophic lateral sclerosis
目的 肌萎缩侧索硬化症(amyotrophic lateral sclerosis,ALS)患者中约90%为散发性,且缺乏明确病因,亟需探索可干预的治疗策略.方法 利用GEO数据库筛选散发性ALS(sporadic ALS,sALS)患者和健康对照的外周血单个核细胞(peripheral blood mononuclear cells,PBMCs)转录组测序数据,随后进行基于炎症水平的分类、差异基因分析和富集分析.结果 根据炎症水平将sALS分为高(H)、中(M)、低(L)三类,其中M和L占比最高(84.4%);差异基因分析显示H组中以先天性免疫激活为主要特征,而M和L较为相似,展示出HIF-1α和白细胞迁移信号通路扰动,另外,L组还表现出额外的EOMES高表达,提示T细胞稳态的失衡.进一步研究发现,HIF-1α在sALS和家族性ALS(familial ALS,fALS)患者的PBMCs中均显著下调,提示其可能是ALS的共同转录特征.在SOD1G93A小鼠模型中,PBMCs中Hif-1α表达水平同样显著下调,支持其在ALS发病机制中的跨物种保守性.结论 本研究探讨了 sALS患者的外周免疫特征,提出新的疾病分类模式,并发现了 HIF-1α和细胞迁移信号扰动的广泛存在,为未来疾病干预提供理论基础.
Objective To investigate the peripheral immune characteristics of sporadic amyotrophic lateral sclerosis(sALS).Methods Bulk RNA-seq data of peripheral blood mononuclear cells(PBMCs)from sALS patients and healthy controls were selected via the GEO database,followed by classification based on inflammation levels,differential gene expression analysis,and enrichment analysis.Results sALS patients were stratified into high(H),medium(M),and low(L)inflammatory subgroups,with M and L subgroups constituting the majority(84.4%).Differential gene analysis revealed that the H subgroup was predominantly characterized by innate immune activation,while M and L subgroups exhibited similarities showing HIF-1α and leukocyte migration signaling perturbation.Additionally,the L subgroup demonstrated unique EOMES overexpression,suggesting T-cell homeostasis imbalance.Further significantly down-regulation of HIF-1α in PBMCs in both sALS and familial ALS(fALS)patients was observed,suggesting it may represent a common transcriptional signature in ALS.This observation is further supported by findings in the SOD1G93A mouse model,where Hif-1 α expression level in PBMCs was similarly markedly reduced,highlighting its cross-species conservation in ALS pathogenesis.Conclusion This study investigates the peripheral immune characteristics of sALS patients,establishes a novel disease stratification framework,and reveales the pervasive dysregulation of HIF-1α and leukocyte migration signaling pathways,laying a robust theoretical foundation for future therapeutic development.
