葶苈子有效成分及治疗心力衰竭研究OA
Effective Ingredient of Tinglizi(Semen Lepidii)and the Treatment of Heart Failure in Clinical Research
葶苈子为十字花科植物播娘蒿(南葶苈子)或独行菜(北葶苈子)的干燥成熟种子,具有泻肺平喘、利水消肿的传统功效.该文系统综述了葶苈子有效成分、药理作用及其在心力衰竭(HF)治疗中的临床应用.目前已明确其主要活性成分包括黄酮类、硫苷类、异硫氰酸类、强心苷类四大类.①黄酮类:已鉴定约60种,如槲皮素、山柰酚、北葶新黄酮D等,其中槲皮素-3-O-β-D-葡萄糖-7-O-β-D-龙胆双糖苷为南葶苈子特有;②硫苷类:如新发现的南葶苈苷;③异硫氰酸类:如3-苯基丙腈等降解产物;④强心苷类:如毒毛旋花子苷元、葶苈苷等.药理研究表明,这些成分通过多靶点、多通路发挥综合效应.黄酮类可靶向HSP90AA1和SRC基因,调控线粒体电子传递及MAPK/PI3K信号通路,改善心肌能量代谢,并通过激活PPARα/PGC-1α/Sirt3通路保护线粒体功能;强心苷类能显著增强心肌收缩力,效果与洋地黄类药物相当;硫苷及异硫氰酸类可抑制肾素-血管紧张素-醛固酮系统(RAAS)活性,调节水通道蛋白表达,发挥利尿消肿作用;此外,其还能通过抑制TGF-β/Smad信号通路改善心肌纤维化,并表现出抗炎、免疫调节、调节血脂及雌激素样等综合作用.在心力衰竭的临床应用中,含葶苈子的复方制剂(如经典方剂葶苈大枣泻肺汤)显示出显著疗效,尤其在慢性心力衰竭患者中,可提升左室射血分数约12.3%,增加6 min步行距离约28.5 m;在风湿性心脏病心力衰竭、顽固性心力衰竭等治疗中,与常规西药联合应用的总有效率可达85%~96.97%;在急性心力衰竭中,能快速改善患者血流动力学状态,降低病死率.该文系统梳理了葶苈子的研究现状,为其深入开发及临床安全应用提供了重要参考依据.
Tinglizi(Semen Lepidii)is the dried mature seed of Descurainia sophia(southern Tinglizi)or Lepidium apetalum(northern Tinglizi)from the Cruciferae family,and it has the traditional effects of purging the lung to relieve asthma and inducing diuresis to reduce edema.This article systematically reviews the research progress of Tinglizi(Semen Lepidii)in terms of its active ingredients,pharmacological effects,and clinical application in the treatment of heart failure(HF).At present,its main active ingredients have been clarified into four categories.①Flavonoids:about 60 types have been identified,such as quercetin,kaempferol,lepidiumflavonol D,etc.,among which quercetin-3-O-β-D-glucose-7-O-β-D-gentiobioside is unique to southern Tinglizi(Semen Lepidii);②Thioglycosides:such as the newly discovered descurainoside;③Isothiocyanates:such as degradation products like 3-phenylpropionitrile;④Cardiac glycosides:such as strophanthidin,lepidiin,etc.Pharmacological studies have shown that these ingredients exert comprehensive effects through multiple targets and pathways.Flavonoids can target HSP90AA1 and SRC genes,regulate mitochondrial electron transport and MAPK/PI3K signaling pathways,improve myocardial energy metabolism,and protect mitochondrial function by activating the PPARα/PGC-1α/Sirt3 pathway;Cardiac glycosides can significantly enhance myocardial contractility,with effects equivalent to digitalis drugs;Thioglycosides and isothiocyanates can inhibit the activity of the renin-angiotensin-aldosterone system(RAAS),regulate the expression of aquaporins,and play a role in diuresis and detumescence;In addition,it can also improve myocardial fibrosis by inhibiting the TGF-β/Smad signaling pathway,and exhibit anti-inflammatory,immune regulation,blood lipid regulation,and estrogen-like effects.In the clinical application for heart failure,compound preparations containing Tinglizi(Semen Lepidii)[such as the classic prescription Tinglizi Dazao Xiefei Decoction(葶苈大枣泻肺汤)]have shown significant efficacy.Especially in patients with chronic heart failure,they can increase the left ventricular ejection fraction by about 12.3%and the 6-minute walking distance by about 28.5 meters;in the treatment of rheumatic heart disease with heart failure and refractory heart failure,the total effective rate can reach 85%-96.97%when combined with conventional western medicine;in acute heart failure,it can quickly improve the hemodynamic status of patients and reduce mortality.This article systematically sorts out the research status of Tinglizi(Semen Lepidii),providing an important reference for its in-depth development and safe clinical application.
