目的 明确电针膝眼穴对膝骨关节炎(knee osteoarthritis,KOA)模型小鼠疼痛敏感性的影响及其机制.方法 选取20只SPF级6月龄C57BL/6J雄性小鼠随机分为空白组、模型组、针刺1组、针刺2组,每组各5只,另取5只SPF级6月龄同类系瞬时受体电位香草酸亚家族4(TRPV4)基因敲除雄性小鼠作为针刺3组,除空白组其余小鼠均在左膝内膝眼注射8 µL胶原酶制剂,右膝内膝眼注射等量0.9%氯化钠溶液,9 d后观察膝关节红肿程度并用游标卡尺测量其肿胀度.针刺1、2、3组均在左侧内外膝眼穴进行电针干预,设置输出脉冲为疏密波,强度分别为1、0.3、1 mA,频率均为2/10 Hz.3组针刺组每次均电针干预15 min,1 d 1次,每周5 d,休息2 d,共干预2周.干预期间模型组小鼠与电针组干预时给予同样的抓取和袜套固定15 min,空白组小鼠不做任何干预.治疗结束后用Von Frey测试记录小鼠左右两足的疼痛阈值.HE染色法观察膝关节软骨组织形态学改变,曼金(Mankin)评分法评价软骨退变程度.结果 与空白组相比,针刺1组和针刺2组小鼠双足疼痛阈值明显提高(P<0.05),并且疼痛阈值均明显高于干预前,分别持续到电针干预后的第12天、第5天(P<0.05);与模型组比较,针刺1组和针刺2组小鼠双足疼痛阈值明显提高(P<0.05),并且疼痛阈值均明显高于干预前,分别持续到电针干预后的第12天、第5天(P<0.05),软骨表层光滑,软骨细胞外形正常,Mankin评分明显降低(P<0.05);与针刺2组比较,针刺1组双足疼痛阈值明显提高(P<0.05);与针刺3组比较,针刺1组和针刺2组小鼠双足疼痛阈值明显提高(P<0.05),并且疼痛阈值均明显高于干预前,分别持续到电针干预后的第12天、第5天(P<0.05),软骨表层光滑,软骨细胞外形正常,Mankin评分明显降低(P<0.05).结论 电针干预可明显提高KOA模型小鼠双足的疼痛阈值,有效提高疼痛敏感性并具有持续效应和整体效应,保护软骨减少损伤,其机制与调控TRPV4通道有关.

Objective To clarify the effects of electroacupuncture of knee-eye points on pain sensitivity of knee osteoarthritis(KOA)model mice and its mechanism.Methods Twenty SPF grade 6-month-old C57BL/6J male mice were selected and randomly divided into blank group,model group,acupuncture group 1,acupuncture group 2,5 mice in each group,and 5 SPF grade 6-month-old TRPV4 knockout male mice of the same strain were taken as acupuncture group 3.Except for the blank group,the rest of the mice were injected with 8 μL of collagenase preparation in the inner knee-eye of the left knee,and an equal amount of saline was injected in the inner knee-eye of the right knee,and the swelling of the knee joints was observed after 9 days.After 9 days,the knee joints were observed for redness and swelling and the degree of swelling was measured with vernier calipers.Acupuncture groups 1,2 and 3 were subjected to electroacupuncture intervention at the left inner and outer knee eye points,and the output pulses were set to be sparse and dense waves with intensities of 1 mA,0.3 mA and 1 mA(the arguments mentioned in the discussion),respectively,and frequencies of 2/10 Hz.Each time of electroacupuncture in all three groups,electroacupuncture intervention was carried out for 15 min,once a day for 5 days per week,with a rest period of 2 days,and the intervention was carried out for 2 weeks in total.The mice in the model group were given the same grasping and stockinette fixation for 15 min during the intervention as in the electroacupuncture group,and the mice in the blank group did not have any intervention.The pain thresholds of the right and left feet of the mice were recorded by the Von Frey test at the end of the treatment.Hematoxylin-eosin(HE)staining was used to observe the histomorphologic changes in the cartilage of the knee joint,and the Mankin score was used to evaluate the degree of cartilage degeneration.Results Compared with the control group,the bilateral paw pain thresholds in mice in acupuncture group 1 and acupuncture group 2 were significantly elevated(P<0.05),and both groups'pain thresholds were significantly higher than those before intervention,persisting until day 12 and day 5 after electroacupuncture intervention,respectively(P<0.05);Compared with the model group,the bilateral pain thresholds of mice in acupuncture group 1 and acupuncture group 2 were significantly increased(P<0.05),and the pain thresholds were significantly higher than those before intervention,persisting until the 12th and 5th days after electroacupuncture intervention,respectively(P<0.05);The cartilage surface was smooth,chondrocytes had normal morphology,and the Mankin scores were significantly reduced(P<0.05);Compared with acupuncture group 2,the bilateral pain thresholds in acupuncture group 1 were significantly higher(P<0.05);Compared with the acupuncture 3 group,the pain thresholds in both feet of mice in the acupuncture 1 and acupuncture 2 groups were significantly higher(P<0.05),and the pain thresholds were significantly higher than before the intervention,persisting until the 12th and 5th days after electroacupuncture,respectively(P<0.05),the cartilage surface was smooth,chondrocytes had normal morphology,and the Mankin scores were significantly reduced(P<0.05).Conclusion Electroacupuncture intervention significantly increased the pain threshold of both feet in KOA model mice,effectively improved pain sensitivity and had sustained and overall effects,and its mechanism was related to the modulation of TRPV4 channels to protect cartilage to reduce damage.
