全视网膜光凝术联合玻璃体腔注射雷珠单抗治疗增生性糖尿病视网膜病变的疗效比较OA
Comparative study on the therapeutic efficacy of panretinal photocoagulation combined with intravitreal ra-nibizumab injection for proliferative diabetic retinopathy
目的 探究全视网膜光凝(PRP)与玻璃体腔注射雷珠单抗(RBZ)治疗增生性糖尿病视网膜病变(PDR)的临床比较研究.方法 前瞻性病例对照研究.纳入2022年6月至2023年11月期间就诊的126例PDR患者.其中I型糖尿患者42例,Ⅱ型糖尿病84例,年龄(52.49±3.56)岁(18~80岁).根据治疗方案将患者分为PRP治疗组和联合治疗组(PRP+RBZ),通过基线访视记录参与者的体重、身高、人口统计学、生命体征和病史等.记录并比较研究过程中不良事件发生率;通过荧光素血管造影术比较患者视盘新生血管(NVD)、视网膜其他部位的新生血管(NVE)及总体新生血管量(NVT)减少量.使用相干光层析成像术(OCT)测量中心凹黄斑厚度(CMT).根据ETDRS规程对患者的BCVA进行了测量.结果 研究中共发生了54次不良事件,PRP治疗组7(12.96%)和联合治疗组5(9.61%)的玻璃体出血发生率比较差异无统计学意义(P>0.05),联合治疗组较PRP治疗组眼压升高、全身性不良发应和血管事件的不良发率降低(均P<0.05),联合治疗组较PRP治疗组总体不良发生率降低(P<0.05).联合治疗组较PRP治疗组NVD、NVE和NVT减少的占比升高(均P<0.05);治疗前两组患者的CMT比较差异无统计学意义(P>0.05),第3个月、第8个月和第12个月联合治疗组较PRP治疗组CMT降低(P<0.05);治疗前的基线最佳矫正视力(BCVA)比较差异无统计学意义(P>0.05);治疗后第3个月,两组BCVA比较差异无统计学意义(P>0.05),第8个月和第12个月联合治疗组较PRP治疗组BCVA降低(均P<0.05);联合治疗组较PRP治疗组的显著率和总有效率升高(均P<0.05),联合治疗组较PRP治疗组效果一般率和无效率降低(P<0.05).结论 PRP+RBZ玻璃体内注射联合治疗可提高单一PRP治愈新生血管形成的成功率,并改善视力及减少副作用.
Objective To explore the clinical comparative study of panretinal photocoagulation(PRP)and intra-vitreal injection of Ranibizumab(RBZ)in the treatment of proliferative diabetic retinopathy(PDR).Methods A prospec-tive case-control study.A total of 106 patients with proliferative diabetic retinopathy(PDR)who visited between June 2022 and November 2023 were enrolled.Among them,there were 42 patients with type I diabetes and 84 patients with type II diabetes,with an average age of(52.49±3.56).According to the treatment plan,the patients were divided into PRP group and combined treatment group(PRP+RBZ).All patients signed an informed consent form before proceeding with the study.The weight,height,demographics,vital signs and medical history of the participants were recorded through the baseline visit.The incidence of adverse events during the study was recorded and compared.The patients'neovasculariza-tion of the optic disc(NVD),retinal neovascularization elsewhere(NVE)and total neovascularization through fluorescein angiography(NVT)reduction amount were compared.Optical coherence tomography(OCT)was used to measure central macular thickness(CMT).The patient's best corrected visual acuity(BCVA)was measured according to the early treat-ment diabetic retinopathy study(ETDRS)protocol.Results A total of 54 adverse events occurred in the study.There was no difference in the incidence of vitreous hemorrhage in the PRP treatment group 7(12.96%)and the combination treat-ment group 5(9.61%)(P>0.05).The combination treatment group was compared with the PRP treatment group,in-creased intraocular pressure,systemic adverse reactions and adverse incidence of vascular events decreased(all P<0.05),and the overall incidence of adverse events in the combined treatment group was lower than that in the PRP treatment group(P<0.05).Compared with the PRP treatment group,the proportion of NVD,NVE and NVT decreased in the combined treatment group was higher(all P<0.05).There was no difference in the average macular thickness of the Chinese and Brit-ish visual fields between the two groups before the treatment(P>0.05).At the third month,the macular thickness of the combined treatment group at the 8th and 12th months were lower than that of the PRP treatment group(P<0.05).There was no difference in the baseline BCVA before the treatment(P>0.05).At the third month after the treatment,there was no difference in BCVA between the two groups(P>0.05),and the combined treatment at the 8th and 12th month was lower than the PRP treatment group BCVA(P<0.05).Compared with the PRP treatment group,the significant rate and total ef-fective rate of combined treatment group increased(P<0.05),and the general rate and inefficiency of the combined treat-ment group were lower than that of the PRP treatment group(P<0.05).Conclusions PRP+ranibizumab intravitreal in-jection of ranibizumab combined therapy can increase the success rate of PRP to cure neovascularization,improve vision and reduce side effects.
