目的 探讨动态增强磁共振成像(DCE-MRI)应用于评估乳腺癌新辅助化疗(NAC)疗效的价值.方法 回顾性分析2021年12月—2023年12月102例乳腺癌患者的临床资料,均接受NAC.根据化疗效果分为有效组(n=74)和无效组(n=28).统计两组基础资料及DCE-MRI定量参数[速率常数(Kep)、容量转移常数(Ktrans)、血管外细胞外间隙容积比(Ve)],多因素Logistic回归分析乳腺癌患者NAC治疗无效的危险因素,绘制受试者工作特征(ROC)曲线评估DCE-MRI定量参数预测乳腺癌患者NAC治疗无效的效能.结果 无效组组织学分级为2~3级占比高于有效组,化疗后Ktrans、Kep、Ve高于有效组(P<0.05).多因素Logistic回归分析证实,组织学分级为2~3级以及Ktrans、Kep、Ve升高均为乳腺癌患者NAC治疗无效的独立危险因素(P<0.05).ROC曲线分析证实,Ktrans、Kep、Ve均可用于乳腺癌患者NAC治疗无效的评估,曲线下面积分别为0.916、0.764、0.946.结论 组织学分级为2~3级以及Ktrans、Kep、Ve升高均会影响乳腺癌患者NAC治疗效果,且DCE-MRI定量参数评估乳腺癌患者NAC治疗无效具有一定价值,应引起临床的足够重视.

Objective To investigate the value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in evaluating the response to neoadjuvant chemotherapy(NAC)for breast cancer.Methods A retrospective analysis was conducted on the clinical data of 102 breast cancer patients who received NAC from December 2021 to December 2023.The patients were divided into the effective group(n=74)and the ineffective group(n=28)based on the chemotherapy outcome.The basic data and quantitative parameters of DCE-MRI[rate constant(Kep),volume transfer constant(Ktrans),extravascular extracellular volume fraction(Ve)]of the two groups were statistically analyzed.Multivariate logistic regression was used to analyze the risk factors for the ineffectiveness of NAC treatment in breast cancer patients.The receiver operating characteristic(ROC)curve was drawn to evaluate the efficacy of DCE-MRI quantitative parameters in predicting the ineffectiveness of NAC treatment in breast cancer patients.Results The proportion of patients with histological grade 2-3 in the ineffective group was higher than that in the effective group.After chemotherapy,Ktrans,Kep and Ve were higher in the ineffective group compared with the effective group(P<0.05).Multivariate logistic regression analysis confirmed that histological grade 2-3,and increased Ktrans,Kep and Ve were all independent risk factors for the ineffectiveness of NAC treatment in breast cancer patients(P<0.05).ROC curve analysis confirmed that Ktrans,Kep and Ve could all be used for the assessment of ineffectiveness of NAC treatment in breast cancer patients,with the areas under the curve of 0.916,0.764,and 0.946,respectively.Conclusion Histological grade 2-3,and increased Ktrans,Kep and Ve all affect the efficacy of NAC in breast cancer patients.Moreover,the quantitative parameters of DCE-MRI have certain value in evaluating the ineffectiveness of NAC treatment in breast cancer patients,and should attract sufficient clinical attention.
